摘 要:目的:分析龙岩地区老年人骨密度与血清VitD水平的关系。方法:选取2015年7月-2016年3月于我院骨伤科病房住院的102例老年患者,收集年龄、体重、身高等资料,计算体质指数,采用骨密度仪测定正位1~4腰椎(L1~4)和股骨颈骨密度,据腰椎骨密度分为骨质疏松组和非骨质疏松组,测定血清25-羟基维生素[25(OH)D]水平,并进行相关性分析。结果:骨质疏松组年龄(71.7±7.6)岁,与非骨质疏松组的(69.3±8.2)岁比较,差异无统计学意义(t=1.49,P=0.14>0.05);骨质疏松组BMI为21.8±3.0,与非骨质疏松组的24.2±3.8比较,差异有统计学意义(t=-3.6,P=0.01<0.05);骨质疏松组血清25(OH)D为(38.3±18.2)nmol/L,低于非骨质疏松组的(49.7±21.8)nmol/L,差异有统计学意义(t=-2.87,P=0.005<0.05);血清25(OH)D≤50nmol/L者,骨质疏松组占46.7%(21/45),非骨质疏松组占28.1%(16/57),组间差异有统计学意义(χ~2=3.76,P=0.042<0.05);进一步直线相关分析结果显示,血清25(OH)D水平与L1~4骨密度呈正相关(r=0.23,P=0.02<0.05),但尚不能认为血清25(OH)D水平与股骨颈骨密度存在直线相关(r=0.15,P=0.16>0.05)。结论:老年人血清VitD水平与骨密度有关,且与腰椎骨密度呈正相关。Objective:To analyze the relationship between the elderly serum vitamin D levels and bone mineral density in Longyan.Methods:A total of 102 patients admitted in Orthopedics were selected from July 2015 to March 2016,age,weight height and serum 25-hydroxyvitamin [25(OH)D]levels were collected at the same time,the BMD at lumbar spine(L1~4)and femoral neck was measured by absorptiometry,according to BMD,patients were divided in to two groups:osteoporosis and non-osteoporosis group.Results:The age(71.7±7.6)years in osteoporosis group,while(69.3±8.2)years in non-osteoporotic group,not respectively(t=1.49,P=0.140.05).The BMI 21.8±3.0 in osteoporosis group,while 24.2±3.8 in non-osteoporotic group,respectively(t=-3.6,P=0.010.05).The concentrations of serum 25(OH)D in two groups were(38.3±18.2)nmol/L and(49.7±21.8)nmol/L,respectively(t=-2.87,P=0.0050.05).And there were 46.7%(21/45)and 28.1%(16/57)of patients whose serum 25(OH)D level were≤50 nmol/L in osteoporosis and non-osteoporosis group,respectively(χ2=3.76,P=0.0420.05).Furthermore,the 25(OH)D level was positively correlated with BMD at L1~4(r=0.23,P=0.020.05),but not correlated with BMD at femoral neck(r=0.15,P=0.160.05).Conclusion:Vitamin D status is correlated with BMD at L1~4.
