摘　　要：我国对健康管理的研究起步相对较晚,虽然目前对健康管理效果评价方面有一定研究,但对健康管理模式还需要进一步的探索与研究。本文从健康管理的内涵及目的着手,结合我国当前老人人慢性病健康管理现状,有针对性的进行健康管理模式探讨分析,以期为我国老年慢性病干预事业做出贡献。The study of health management in China started relatively late. Although there are some studies on the evaluation of health management effectiveness, it still needs further exploration and research on the health management model. Starting with the connotation and purpose of health management, combined with the current status of elderly people＇s chronic health management in China,this article conducts targeted analysis of health management model, with a view to making contributions to China＇s elderly chronic disease intervention.
