单孔与多孔腹腔镜双侧输卵管结扎术及其术后体外受精胚胎移植结局的对比分析

摘　　要：目的对比经脐单孔腹腔镜(LESS)与多孔腹腔镜行双侧输卵管结扎术的围手术期资料,术后行体外受精-胚胎移植术(IVF-ET)助孕的临床结局。方法 2016年1月至2017年12月双侧输卵管积水患者经脐LESS双侧输卵管结扎术32例,多孔腹腔镜双侧输卵管结扎术60例,对比两组手术时间、出血量、术后住院时间;术后IVF-ET,窦卵泡数(AFC)、抗苗勒氏管激素(AMH)、促性腺激素(Gn)总量、Gn天数、获卵数、优胚率、种植率、妊娠率、早期流产率。结果 LESS组与多孔组相比,手术时间延长(P<0.001)、住院时间缩短(P<0.001)。LESS组与多孔组术中出血量比较,差异无统计学意义(P=0.40)。LESS组与多孔组术后IVF-ET,其AFC、AMH、Gn总量、Gn天数、获卵数、优胚率、种植率、妊娠率、早期流产率差异均无统计学意义(P>0.05)。结论经脐LESS双侧输卵管结扎术安全可行。Objective To compare the parameters of bilateral tubal ligation by LESS and multi-incisionlaparoscopy and their prognosis of IVF-ET. Methods From Jun. 2016 to Dec. 2017,32 patients underwent LESSbilateral tubal ligation and 60 patients under-went multi-incision laparoscopic bilateral tubal ligation were enrolled.The demographic data,operative parameters,and postoperative prognosis of IVF-ET in the two groups weresummarized and compared. Results The mean operating time in LESS group was prolonged and its operativehospital stay was significantly decreased compared to multi-incision laparoscopic group. No significant differencewas observed in the intraoperative blood loss in two groups. Moreover,no significant differences were found in theAFC,AMH,Gn stimulation days,total Gn dosage,Ovum number,good quality embryo rate,embryo implantionrate,clinical pregnancy rate and early spontaneous abortion rate in two groups. Conclusions For experiencedlaparoscopic surgeons,LESS is a safe and feasible procedure with similar perioperative outcomes to multi-incisionlaparoscopic surgery.
