摘 要:目的探讨延续性护理在脑电双频指数联合振幅整合脑电图判断患儿昏迷程度中的护理效果。方法选择2012年5月~2016年10月入院治疗的昏迷患儿140例,根据护理方法不同分为对照组70例和观察组70例。患儿均采用脑电双频指数联合振幅整合脑电图评估昏迷程度。对照组采用常规方法护理,观察组采用延续性护理,比较两组护理效果及对预后的影响。结果观察组护理后BIS及GCS评分,高于对照组(P<0.05);两组护理前a EEG连续性、睡眠觉醒周期及下边缘振幅值评分比较,差异无统计学意义(P>0.05);观察组护理后a EEG连续性、睡眠觉醒周期及下边缘振幅值评分,高于对照组(P<0.05);观察组护理后康复率,高于对照组(P<0.05);观察组护理后残疾、植物人及死亡率,低于对照组(P<0.05)。结论采用脑电双频指数联合振幅整合脑电图能准确的评估患儿昏迷程度,评估时加强患儿延续性护理有助于提高护理质量,改善患儿预后,值得推广应用。Objective To investigate the effect of continuous nursing in the diagnosis of coma in children using bispectral index combined with amplitude integrated electroencephalography. Methods 140 cases of coma children admitted to hospital from May 2012 to October 2016 were selected and divided into control group(70 cases) and observation group (70 cases) according to different nursing methods. All children were assessed for the degree of coma using bispectral index combined with amplitude integrated electroencephalography. The control group was treated with conventional nursing and the observation was given continuous nursing. We compared the effect of nursing in two groups and the effect on prognosis. Results The BIS and GCS scores in the observation group were higher than those in the control group after nursing (P〈0.05). There was no significant difference in aEEG continuity, sleep-waking cycle and lower edge amplitude scores of the two groups before nursing (P〉0.05). The scores of aEEG continuity, sleep-waking cycle and lower edge amplitude after nursing were higher in the observation group than in the control group (P〈0.05). The recovery rate after nursing in the observation group was higher than that in the control group (P〈0.05). The disability, vegetable and mortality rates after nursing in the observation group were lower than those in the control group (P〈0.05). Conclusion The use of bispectral index combined with amplitude integrated electroencephalography can accurately assess the degree of coma in children. Strengthening the continuity of nursing care during assessment can help improve the quality of care and improve the prognosis of children. It is worthy of promotion and application.
