摘 要:目的对乳腺癌患者外周血标本进行蛋白质水平筛查分析,明确女性乳腺癌发生及绝经前后与IGFBP-2和FGA表达调控的关系。为建立乳腺癌早期预警及临床治疗疗效评估指标提供依据。方法收集病理确诊为乳腺癌的患者和健康志愿者外周血标本共85例,其中绝经后患者29例,未绝经患者36例,正常对照20例。采用酶联免疫吸附试验(ELISA)方法,检测外周血(血浆)胰岛素样生长因子结合蛋白2( insulin-like growth factor binding protein 2, IGFBP2)和纤维蛋白原α链( fibrinogen alpha chain, FGA)含量。结果绝经后乳腺癌患者体内IGFBP-2含量升高,与未绝经患者和正常对照有显著差异(P〈0.05)。正常对照组患者血浆FGA含量低于未绝经及绝经后乳腺癌患者(P〈0.05)。未绝经乳腺癌与绝经后乳腺癌患者之间,FGA含量也存在显著差异(P〈0.05)。受信者操作特性( receiver operating characteristic, ROC)曲线分析显示,血浆IGFBP-2和FGA表达水平均有较高的灵敏度和特异性。结论绝经后和未绝经乳腺癌患者伴有血浆IGFBP-2与FGA含量升高,可能与乳腺癌IGFBP-2、FGA蛋白质的表达调控异常存在内在联系,此为进一步揭示乳腺癌发病机制、早期预警和临床治疗疗效评估提供依据。Objective s: The protein levels of peripheral blood from patients with breast canc- er were screened and analyzed, to determine the relationship between breast cancer and the expres- sion of IGFBP-2 and FGA of post-menopausal and non-menopausal women, and provide basis for the establishment of early warning of breast cancer and evaluation index of clinical treatment effiea-cy. Methods A total of 85 peripheral blood samples were collected from pathologically diagnosed breast cancer patients and healthy volunteers, including 29 cases of postmenopausal patients, 36 cases of non-menopausal patients, and 20 cases of normal controls. The levels of insulin-like growth factor binding protein 2 (IGFBP2) and fibrinogen alpha chain (FGA) in peripheral blood (plas- ma) were determined by enzyme-linked immunosorbent assay (ELISA) . Results As compared with the normal control group, the plasma level of IGFBP-2 was increased significantly in postmeno- pausal breast cancer group (P 〈 0. 05 ) . However, there was no difference in plasma IGFBP-2 lev- el between the non-menopausal breast cancer group and the normal control group (P 〉 0.05 ) . There was also a significant difference in plasma IGFBP-2 level between non-menopausal women with breast cancer and postmenopausal women with breast cancer (P 〈 0.05 ) . Plasma FGA level was lower in the normal control group than in the non-menopausal and postmenopausal breast cancer patients ( P 〈0. 05 ) . There was also a significant difference in FGA content between non-menopa- usal women with breast cancer and postmenopausal women with breast cancer (P 〈 0.05) . The Receiver Operating Characteristic (ROC) curve analysis showed that the use of ELISA to determine IGFBP-2 and FGA expression levels had higher sensitivity and specificity. Conclusion The occur- rence of female breast cancer is related to abnormal levels of IGFBP-2 and FGAin vh^o.
