冠状动脉CTA在降主动脉弹性功能研究中的价值

摘　　要：目的通过冠状动脉CT血管造影（CTA）检查,分析降主动脉的弹性特点,并研究其早期检测动脉弹性变化的可行性及临床意义。方法选取行冠状动脉CTA检查的86例健康者作为研究对象（女38例,男48例）,按年龄分为3组（A组：〈40岁;B组：≥40~≤50岁;C组：〉50岁）。将原始数据间隔5%RR间期进行重建,并测量每一时相肺动脉分叉处降主动脉的平均直径与横截面积,计算出4个弹性指标（%Ao、AoD、AoC、AoSI）,分析三组间健康人群降主动脉的搏动规律及弹性特点。结果降主动脉达到最大直径及最小直径时的RR时相分别为（33.72±6.19）%及（99.09±6.14）%,与心率无明显相关性（P〉0.05）;A、B、C组直径变化分别为（2.05±0.42）mm、（1.76±0.51）mm及（1.61±0.39）mm。结论降主动脉在整个心动周期中产生规律性搏动,其弹性与年龄呈负相关。冠状动脉CTA可以早期检测动脉弹性变化,对预防心血管疾病的发生具有重要临床意义。Objective To analyse the elastic characteristics of the descending aorta,and study feasibility and clinical application value in the evaluation of arterial elastic function change early,through coronary CTA examination. Methods86 healthy subjects（ male 48 cases,female 38 cases） were selected in First Affiliated Hospital of Jiamusi University for coronary CTA examination as the object of study,were divided into 3 groups according age（ group A, 〈40 years; group B： 40~ 50 years; C group： 〉50 years）. Original data was reconstructed in 5% steps throughout the RR interval. The diameter and area of the plane of interest were measured,and the 4 elastic index（ % Ao,AoD,AoC,AoSI）,were calculated. Then the pulsatile rule and elastic characteristics of descending aorta were analyzed. Results The descending aorta reached the maximum diameter and the minimum diameter at the RR phase of（ 33. 72 ± 6. 19） % and（ 99. 09 ± 6. 14） %,there was no significant correlation with heart rate（ P〉0. 05）. The diameter changes of A,B and C groups were（ 2. 05 ± 0. 42） mm,（ 1. 76 ± 0. 51） mm and（ 1. 61 ± 0. 39） mm. Conclusion The descending aorta with regular rhythm of the cardiac pulsation and its elasticity decreases with age. Coronary CTA could detect arterial elasticity change early,and maybe has important clinical value to prevent the occurrence of cardiovascular disease.
