正常小儿及先天性肛门直肠畸形患儿肛周肌群的MRI应用研究

摘　　要：目的探讨MRI在正常小儿及先天性肛门直肠畸形中评估肛周肌群的应用价值。方法分析25例无直肠肛管疾病小儿及37例确诊为先天性肛门直肠畸形患儿的盆底MRI表现,观察肛提肌及肛门括约肌发育情况。结果无直肠肛管疾病儿童肛提肌及肛门括约肌均显示良好,37例先天性肛门直肠畸形患儿中,肛门括约肌中23例发育差（肌腹细小、两侧不对称、形态不规则）,8例发育尚可,6例发育好。肛提肌中15例发育差（肌腹纤细、两侧不对称、形态欠佳）,9例发育尚可,13例发育好。结论 MRI可清晰显示肛提肌及肛门括约肌的发育情况,且安全无辐射,为先天性肛门直肠畸形患儿制定手术方案及评估预后提供重要的影像学支持。Objective To investigate the value of magnetic resonance imaging in the evaluation of perianal muscles in normal children and congenital anorectal malformations. Methods MRI was performed in 25 cases children without anorectal diseases and 37 cases children with surgically proved congenital anorectal malformation,the development of levator ani muscle and anal sphincter muscle were analyzed. Results The development of levator ani muscle and anal sphincter muscle in children without anorectal diseases could be demonstrated well using MRI. Of the 37 cases,there were 23 cases of poor development of anal sphincter muscle,including with slender muscle belly,bilateral asymmetry,irregular shape,8 cases developed near-normal,6 cases of good development. There were 15 cases of poor development of levator ani muscle,including with slender muscle belly,bilateral asymmetry,shape not perfect,9 cases developed near-normal,13 cases of good development. Conclusion MRI can clearly show the development of levator ani muscle and anal sphincter,and it is safe and radiation-free. It provides important imaging support for the development of surgical plan and prognosis for children with congenital anorectal malformation.
