中青年人群右侧锁骨下动脉内膜超声检查提示早期动脉硬化的临床价值研究

摘　　要：目的探讨超声检查中青年人群右侧锁骨下动脉内膜增厚性病变提示早期动脉硬化的临床价值。方法选取在本院体检中行颈部血管超声检查,同时行右侧锁骨下动脉超声检查的中青年324例,年龄(39.8±6.6)岁,对右锁骨下动脉起始处后壁及双侧颈总动脉中远段、颈总动脉分叉处及颈内、外动脉起始处进行检测,分别观察内膜是否增厚、斑块有无及性质,彩色或能量多普勒了解血流束充盈情况,充盈是否缺损、血管有无狭窄或闭塞。结果右侧锁骨下动脉内膜增厚性改变但无颈动脉内膜中层增厚(intima-media thickness,IMT)改变(IMT≥0.1cm定义为内中膜增厚,≥0.15cm定义为斑块)的140例(140/324,43.2%);右侧锁骨下动脉内膜增厚性改变同时伴有颈动脉IMT增厚性改变的61例(61/324,18.8%);右侧锁骨下动脉IMT及颈动脉IMT均正常的119例(119/324,36.7%);单纯颈动脉内膜增厚4例(4/324,1.3%)。结论 324例中青年人右侧锁骨下动脉内膜增厚性改变病例数远远高于颈动脉IMT增厚性改变的病例数,颈动脉IMT增厚改变病例数仅占总病例的1/5,而右侧锁骨下动脉内膜增厚性改变的病例数高达61.7%,同时在右侧锁骨下动脉内膜增厚性改变病例中发现颈动脉IMT增厚病例数还不到20.0%,因此,超声检查发现中青年人群中右侧锁骨下动脉内膜增厚性改变较颈动脉IMT更早期提示动脉硬化,并量化评估其程度。Objective To investigate the value of ultrasound examination of intima-media thickening of right subclavian artery in young and middle-aged people hint early atherosclerosis.Methods From chose 324 young and middleaged people,ages（39.8±6.6）years old,in this hospital Outpatient Department and/or health examination center,examine carotid and right subclavian artery by ultrasound,the right subclavian artery and bilateral carotid artery distal and bilateral carotid artery bifurcation,bilateral internal and external artery were detected,including intima-media thickening,plaque,Color Doppler or Power Doppler ultrasound understand the blood filling defect and no beam vascular stenosis and occlusion.Results Right subclavian artery intimal thickening but no change of carotid intima-media thickness（IMT）change（IMT≥0.1 cm is defined as the intima-media thickness,plaque was defined as more than or equal to 0.15 cm）of 140 cases（140/324,43.2%）;the right subclavian artery intimal thickening and 61 cases with carotid artery IMT thicken（61/324,18.8%）;119 cases of the right subclavian artery IMT and carotid artery IMT were normal（119/324,36.7%）;alone carotid IMT thicken in 4 cases（4/324,1.3%）.Conclusions Right subclavian artery intimal thickening change the number of cases is much higher than the number of cases of carotid IMT thickening in 324 young and middle-aged people,carotid artery IMT changes the number of cases accounted for only 1/5 of the total cases,the number of cases of the right subclavian artery intimal thickening change as high as 61.7%,while in the right subclavian artery intima thickening cases found the carotid artery IMT changes were less than 20.0%,therefore,the right subclavian artery intimal thickening of the ultrasound examination showed that young and middle-aged people can change more early than carotid IMT suggest that atherosclerosis,the degree of evaluation and quantization.
