摘  要:目的探讨应用经会阴实时三维超声评估不同分娩方式对经产后女性盆底结构的近期影响。方法选取115例经产妇(经阴道分娩组70例,剖宫产组45例)于产后6~8周行盆底实时三维超声检查,观察与测量以下指标:静息状态下膀胱颈位置及肛提肌裂孔面积、最大Valsalva动作下的膀胱颈移动度(BND)、尿道内口有无形成漏斗状、膀胱有无膨出及肛提肌裂孔面积,比较两组间的差异。结果经阴道分娩组的膀胱颈移动度、膀胱膨出率、两种状态下的肛提肌面积均高于剖宫产组,差异有统计学意义(P均<0.05),两组间在膀胱颈位置及尿道内口漏斗形成率上无统计学意义(P>0.05)。结论实时三维超声能够有效观察产后女性盆底结构,阴道分娩对女性盆底结构的近期影响大于选择性剖宫产。Objective To observe the early influence of different delivery modes on multipara pelvic floor structures by using transperineal real time three-dimensional ultrasound.Methods 115 multiparaes including 70 cases with vaginal delivery group and 45 cases with cesarean delivery group were recruited and underwent the transperineal real time three-dimensional ultrasound in 6-8 weeks after childbirth.The parameters of resting and Valsalva condition,including bladder neck position,levator hiatus area,bladder neck descent(BND),the existence of internal urethral orifice funnel and vesicocele were respectively observed and measured.Results The BND and the rate of vesicocele and the levator hiatus area in the vaginal delivery group were lager than in the cesarean delivery group,the differences were statistically significant(P〈0.05).There was no significant difference in the bladder neck position and the rate of internal urethral orifice funnel between the two groups in resting state(P〉0.05).Conclusions Real time three-dimensional ultrasound can effectively observe postpartum female pelvic floor structure.The early influence on multipara pelvic floor structures in vaginal delivery is more than in selective cesarean section.
