EH病人心率变异特征与血脂、C反应蛋白、心肌酶及肾功能的相关性

摘　　要：目的了解原发性高血压(EH)患者心率变异性(HRV)特征,探讨HRV与血脂、C反应蛋白(CRP)、心肌酶及肾功能的相关性。方法选取广东省佛山市南海区第六人民医院收治的88例初发未治疗EH患者为病例组,88例同期该院健康体检者为对照组。比较两组HRV、CRP、血脂、肾功能、心肌酶等资料,并分析EH患者HRV和其他资料的相关性。结果与对照组相比,病例组HRV指标:RR间期标准差(SDNN)、RR间期平均值标准差(SDANN)、相邻RR间期差值均方根(RMSSD)、相邻正常RR间期之差>50 ms的百分比(PNN50)均较低(P<0.05),而三酰甘油(TG)、CRP、血尿素氮(BUN)、尿糖(GLU)较高(P<0.05);相关性分析发现,EH患者TG与SDNN、SDANN、RMSSD弱相关,CRP、GLU与SDNN、SDANN中等相关、与PNN50弱相关,天门冬氨酸氨基转移酶(AST)与RMSSD中等相关,颈动脉斑块数与SDNN、SDANN中等相关(P<0.05)。结论 EH患者HRV指标下降,并与靶器官损伤程度之间存在相关性,HRV指标值可为高血压的诊断和治疗提供参考。Objective To investigate the characteristics of heart rate variability（HRV）of essential hypertension（EH）patients,and analyze the correlation between HRV and blood lipids,C reactive protein（CRP）,myocardial enzymes and renal function.Methods A total of 88 EH patients from the Sixth People′s Hospital of Nanhai District were selected as case group;88 healthy adults undergone physical examination at the same period in this hospital were taken as control group.The differences of HRV,CRP,serum lipids,myocardial enzymes and renal function in two groups were analyzed.The correlations between HRV and these indexes were estimated by Pearson test.Results Compared with the control group,HRV indicators：standard deviation of NN intervals（SDNN）,standard deviation of all NN intervals（SDANN）,root mean squared suecessive diference（RMSSD）,percent of NN 50 in the total number NN intervals（PNN50）of the case group were lower（P〈0.05）;triglyceride（TG）,CRP,blood urea nitrogen（BUN）,glucosuria（GLU）were higher（P〈0.05）in the case group than those of control group.In the EH patients group,TG was weakly correlated with SDNN,SDANN and RMSSD.CRP and GLU was moderately correlated with SDNN and SDANN,and weakly correlated with PNN50.Aspartate aminotransferase（AST）was moderately correlated with RMSSD.And carotid plaque number was moderately correlated with SDNN and SDANN（all P-values0.05）.Conclusion HRV indexes were significantly reduced in EH patients.There was a correlation between HRV indexes changes and target organ damage.HRV indexes could provide helpful information for the diagnosis and treatment of EH.
