下颌前磨牙V型根管的CBCT研究

摘　　要：目的:采用CBCT探讨下颌前磨牙V型根管的发现率及牙根和根管系统解剖形态特征。方法:回顾148例患者288颗下颌第一前磨牙和218颗下颌第二前磨牙的CBCT影像,采用Vertucci(1984)提出的分类方法,诊断出V型根管的下颌前磨牙,计算发现率,研究牙根和根管系统形态特征及其对称性。结果:下颌第一和第二前磨牙V型根管的发现率分别为14.58%(42/288)和1.83%(4/218)。46颗V型根管下颌前磨牙中有根面沟36颗(78.26%),根面沟多发生在近中面偏舌侧。根管分歧发生于根中1/3的占91.30%。根管分布绝大多数为颊舌向双根。18颗下颌前磨牙的根管构型对称性分布。结论:下颌第一前磨牙V型根管的发现率远高于下颌第二前磨牙。多数牙根有根面沟并且与双根管构型有一定关系。根管分歧大多数发生在根中1/3处。根管构型分布有一定的对称性。Objective： To investigate the morphological characteristics of Vertucci type Ⅴ canal configuration in mandibular premolars using CBCT. Methods： CBCT images of 288 mandibular first premolars and 218 mandibular second premolars in 148 patients were retrospectively analyzed. The mandibular premolars with Vertucci type Ⅴ canal configuration were numbered. The root morphology,the canal configuration and bilateral symmetry were analyzed. Results： The incidence of Vertucci type Ⅴ canal configuration was14. 58%（ 42/288） in mandibular first premolars and 1. 83%（ 4/218） in mandibular second premolars. 78. 26% mandibular premolars had radicular grooves,and they usually located at mesio-lingual side. 91. 30% root canal bifurcation was at the middle third in most bicanal mandibular premolars. 18 cases of Vertucci type Ⅴ canal configuration showed bilateral symmetry. Conclusion： The mandibular first premolars have a higher incidence of Vertucci type Ⅴ canal configuration than the second premolars. Most mandibular premolars have radicular grooves. And the radicular grooves are related to 2 canals. Root canal distribution have a certain bilateral symmetry.
