摘  要:目的检测人巨细胞病毒(HCMV)编码的微小核糖核酸miR-UL112-3p(hcmv-miR-UL112-3p)在口腔扁平苔藓患者血浆中的水平,探讨其潜在的临床价值。方法收集74例口腔扁平苔藓患者血液样本,包括13例全血标本和61例血浆标本;同时收集年龄、性别匹配的健康人全血标本12例和血浆样本51例。用实时荧光定量PCR(qRT-PCR)法测定全血标本中外周血白细胞HCMV DNA滴度;ELISA法测定血浆中抗HCMV Ig G和Ig M抗体含量;用基于TaqMan探针的qRT-PCR技术分别在初筛人群(患者21例,对照18例)和验证人群(患者40例,对照33例)中测定血浆样本中hcmv-miR-UL112-3p水平。结果口腔扁平苔藓患者外周血白细胞中HCMV DNA滴度显著高于健康人对照组[(17 319±15 311)copies/m L vs(2 672±2 368)copies/m L,t=3.273,P<0.01]。两组间血浆抗HCMV Ig G(100%vs 93.65%)和Ig M抗体阳性率(0 vs 0)差异无统计学意义。初筛人群中hcmv-miR-UL112-3p在患者组中的水平[3.531(1.969,4.691)]显著高于健康人对照组[0.450(0.303,0.722)](U=2.0,P<0.01);验证人群中患者组hcmv-miR-UL112-3p水平[1.348(0.964,2.593)]显著高于对照组[1.039(0.803,1.549)](U=441.0,P<0.05)。多元logistic回归显示,hcmv-miR-UL112-3p与口腔扁平苔癣疾病相关(OR=6.00,P<0.01)。结论hcmv-miR-UL112-3p在口腔扁平苔藓患者血浆中显著升高,可能是口腔扁平苔藓疾病潜在的血清学指标,值得进一步深入研究。Objective To investigate the levels of miR-UL112-3p encoded by human cytomegalovirus( HCMV) in blood plasma of patients with oral lichen planus( OLP) and its potential clinical value. Methods The blood samples from 74 OLP patients,including 13 whole blood samples and 61 blood plasma samples,and from 63 age/sex-matched healthy controls,including 12 whole blood samples and 51 blood plasma samples,were collected. The titers of HCMV DNA in peripheral blood leukocytes from the whole blood samples and the levels of anti-HCMV Ig G and Ig M in blood plasma samples were determined by q RT-PCR and ELISA kits,respectively. The levels of miR-UL112-3 p in blood plasma samples from the preliminary screening cohort,including 21 OLP patients and 18 healthy controls,and from the validation cohort,including 40 OLP patients and 33 healthy controls,were detected by the Taq Man q RT-PCR assay. Results The titers of HCMV DNA in peripheral blood leukocytes of OLP patients( 17 319±15 311 copies/m L) were significantly higher than that in healthy controls( 2 672±2 368 copies/m L,t = 3.273,P〈0.01). However,there was no significant difference in the positive rates of anti-HCMV Ig G( 100% vs 93.65%) and anti-HCMV Ig M( 0 vs 0) between the two groups. In the preliminary screening cohort,the plasma levels( median [P25,P75]) of miR-UL112-3 p in OLP patients( 3.531 [1.969,4.691]) were significantly higher than that in healthy controls( 0. 4503 [0. 303,0. 722],U = 2. 0,P〈0. 01). In the validation cohort,the plasma levels of miR-UL112-3 p in OLP patients( 1.348 [0.964,2.593]) were also significantly higher than that in healthy controls( 1.039 [0.803,1.549],U = 441.0,P〈0.05). The multiple logistic regression analysis showed that the increased miR-UL112-3 p levels were correlated with OLP( OR = 6.00,P〈0. 01). Conclusion The plasma levels of miR-UL112-3 p encoded by HCMV in OLP patients increase significantly,indicating that it may be the potential serological marker of OLP.
