米力农治疗老年肺心病心力衰竭临床观察

摘　　要：目的观察米力农治疗老年肺心病心力衰竭患者的疗效、安全性及对心功能、血管内皮功能的影响。方法将85例肺心病心力衰竭老年患者随机分为观察组43例和对照组42例,对照组予常规治疗,观察组常规治疗加用米力农治疗,10 d为1个疗程。比较2组临床疗效、心功能、动脉血气分析和内皮功能,记录2组不良反应。结果观察组患者的总有效率为81.40%明显高于对照组患者的61.90%,差异有统计学意义(P<0.05)。治疗前2组心功能指标比较差异无统计学意义(P>0.05)。2组患者LVEDD、LVESD治疗前后比较差异均无统计学意义(P>0.05)。治疗后2组患者的LVEF、SV、CO增高,NT-pro BNP含量降低,且观察组变化幅度大于对照组,差异均有统计学意义(P<0.05)。治疗前2组动脉血气分析和内皮功能比较差异无统计学意义(P>0.05)。治疗后2组患者PO2、NO增高,PCO2、ET-1降低,且观察组上述指标变化幅度大于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率为9.3%(4/43)高于对照组的7.1%(3/42),但组间比较差异无统计学意义(P>0.05)。结论米力农治疗老年肺心病心力衰竭安全有效,且可改善心功能、动脉血气,保护血管内皮功能。Objective To observe the clinical effects and safety of milrinone in treatment of elderly patients with pulmonary heart disease and heart failure,and its impacts on heart function and vascular endothelial function. Methods 85 cases were of patients with pulmonary heart disease and heart failure randomly divided into study group 43 cases and control group 42 cases. Control group received conventional treatment,study group received milrinone additionally,all treated for 10 days. Clinical efficacy,heart function,arterial blood gas analysis and the adverse reactions of two groups were recorded. Results The total effective rate of observation group was 81. 40%,which was significantly higher than that of control group（ 61. 90%）,the difference was statistically significant（ P〈0. 05）. There was no significant difference in the cardiac function indexes between two groups before treatment（ P〉0. 05）. There were no significant differences between two groups in the diagnosis of LVEDD and LVESD before and after treatment（ P〉0. 05）. After treatment,the LVEF,SV,CO increased,NT-pro BNP content decreased,and the change of observation group was greater than that of control group,the difference was statistically significant（ P〈0. 05）. There was no significant difference in arterial blood gas analysis and endothelial function between two groups before treatment（ P〉0. 05）. After treatment,and PO2 and NO increased,PCO2 and ET-1 decreased,and the change of the above indexes in observation group was larger than that of control group,and the difference was statistically significant（ P〈0. 05）.The incidence of adverse reactions was 9. 3%（ 4/43） in observation group and 7. 1%（ 3/42） in control group,but there was no significant difference between two groups（ P〉0. 05）. Conclusion Milrinone is effective and safe for elderly patients with pulmonary heart disease and heart failure,and could improve heart function and arterial blood gas,and protect vascular endothelial function.
