扬州市江都区追踪肺结核病人就诊影响因素分析

摘　　要：目的:分析需追踪肺结核病人特征与需追踪原因,提高结核病人及时转诊到位率。方法:分析2014-2016年江都区需追踪肺结核病人特征,对2015-2016年需追踪病人进行问卷调查。结果:2014-2016年江都区转诊到位率分别为87.33%、93.18%、90.79%;3年转诊到位率差别有统计学意义(P<0.01);需追踪病人性别差异无统计学意义(P>0.05);年龄别差异有统计学意义(P<0.05);不同职业者差异有统计学意义(P<0.01);乡镇间差异无统计学意义(P>0.05);月份间需追踪率差异有统计学意义(P<0.01)。影响及时转诊到位因素为行动不便、没有就诊时间、自认为没必要治疗、担心社会歧视家人困扰、不信任区定点机构、拒绝就诊等。结论:进一步加大结核病防治知识宣传,提高家人在老年患者及时就诊中的作用,提高基层医生解释引导能力,增加基层结核病定点诊疗机构。Objective To analyze the epidemiological characteristics of TB patients that need to be tracked and the reasons for tracking them so as to provide reference for improving timely referral rate among TB patients.Methods The epidemiological characteristics of TB patients needed to be tracked in Jiangdu district from 2014 to 2016 were analyzed and a questionnaire survey was conducted among patients tracked during 2015-2016.Results In 2014-2016,the referral rates of TB patients in Jiangdu district were 87.33%,93.18% and 90.79%,respectively.There was a statistically significant difference in referral rate（P〈0.01）.There were no statistically significant differences in patient＇s gender and towns（both P〉0.05）.Yet there were statistically significant differences in age,profession（P〈0.05,P〈0.01）.And there was a statistically significant difference in referral rate between months（P〈0.01）.The factors that affected timely referral were having difficulty in moving,having no time to visit a doctor,thinking it unnecessary to treat,worrying about social discrimination and family concerns,distrusting the designated medical institution of the district,and refusing to visit a doctor,etc.Conclusions It is necessary to further promote the publicity of TB prevention and control,enhance the family member＇s role in the timely treatment of elderly patients,improve the interpretation and guiding ability of doctors in primary medical institutions and increase the number of designated TB medical institutions.
