2011-2016年天台县肾综合征出血热监测分析

摘　　要：目的:探讨天台县肾综合征出血热(HFRS)的流行特征和发展趋势,为HFRS的精准防控提供科学依据。方法:使用统一的个案调查表对所有HFRS临床诊断病例进行个案调查,并采集样本采用间接免疫荧光法检测患者血清抗体。采用夹夜法捕获鼠形动物,无菌操作取肺标本后,采用直接免疫荧光法检测鼠肺汉坦病毒(HV)抗原。结果:2011-2016年全县累计报告HFRS病例245例,年平均发病率6.88/10万;男女发病之比为1.75∶1(156/89),以农民发病为主,占90.2%。室内鼠密度平均为1.73%,野外为5.01%,平均鼠密度为4.30%,鼠带病毒率平均为4.56%。结论:天台县是以姬鼠型为主的混合型HFRS疫区,需要进一步加大监测和预防力度。Objective To explore the epidemiological characteristics and development trend of hemorrhagic fever with renal syndrome（HFRS）in Tiantai county so as to provide reference for strategies specific to the prevention and control of the epidemic.Methods A standard questionnaire was used to investigate epidemiological characteristics of clinically diagnosed HFRS cases.Serum samples were collected from these individuals and Hantaan virus antibodies were detected using indirect immunofluorescence assay（IFA）.Small mammals were captured using traps and lung samples were collected from captured small mammals.The virus carrying state of the host animals was determined by Hantaan virus antigen detection with the direct immunofluorescence assay.Results A total of 245 HFRS cases were reported with an average annual incidence rate of 6.88 per 100,000 in Tiantai county,2011-2016.There was an incidence ratio of 1.75∶1 for male and female patients,with farmers affected predominantly accounting for 90.20%.The average density of rodents was 4.30% with an average indoor rodent density of 1.73% and an outdoor one of 5.01%.The average virus carrying rate of rodents was 4.56%.Conclusions Tiantai county is a mixed HFRS epidemic area,with the Apodemus type dominant,therefore,greater efforts need to be made in the surveillance and prevention of the epidemic.
