头、体针联合应用分期治疗缺血性中风患者运动功能障碍的临床研究

摘　　要：目的:探讨头、体针联合应用分期治疗缺血性中风(软瘫期、痉挛期)患者运动功能障碍的疗效。方法:88例纳入观察的患者随机分为治疗组和对照组,每组44例。两组均接受常规药物、康复治疗,治疗组采用头、体针联合应用分期针刺;对照组按照"醒脑开窍"针刺法不分期选穴针刺治疗,均连续治疗4周。采用改良的FuglMeyer运动功能评分法(FMA)、Barthel指数(BI)、10 s抓握测试(HGRT)、FAC功能性步行量表、肱三头肌和胫骨前肌肌电积分(i EMG)对两组患者治疗前后的运动功能及日常活动能力进行评定。结果:两组患者治疗后FMA、BI、HGRT、FAC、肱三头肌和胫骨前肌的i EMG评分均有明显提高(P<0.05),且治疗后治疗组的FMA、BI、HGRT、FAC、肱三头肌和胫骨前肌的i EMG评分均高于对照组(P<0.05)。结论:头、体针联合应用分期治疗缺血性中风(软瘫期、痉挛期)可明显提高患者的运动功能和日常生活能力,改善步行功能和手功能,增强患侧肢体抗痉挛肌肌张力。Objective： The combined application of scalp and body acupuncture by stages for motor dysfunc- tion of patients with ischemic stroke （flaccid paralysis, convulsion period）. Methods： A total of 88 patients were randomly divided into the observation group and the control group, with 44 cases in each group. Two groups were treated with routine medicine and rehabilitation treatment for 4 consecutive weeks. The observation group was treated with scalp and body acupuncture by stages, while the control group was treated with the ＂acupunc- ture method of inducing resuscitation＂. The motor function and daily activity of the two groups before and after treatment were compared by FMA, BI, HGRT, FAC and iEMG. Results： After treatment, FMA, BI, HGRT, FAC, triceps brachii and tibialis anterior muscle iEMG score of patients were significantly improved （P〈0.05）, and FMA, BI, HGRT, FAC, triceps brachii and tibialis anterior muscle iEMG score in the observation group were higher than those in the control group （P〈0.05）. Conclusion： The combined application of scalp and body acupuncture by stages therapy can significantly improve motor function and ability of daily life for patients with ischemic stroke （flaccid paralysis, convulsion period）, improve walking function and hand function, enhanced anti spasm limbs muscular tension, and thus it is worthy of clinical application.
