血小板减少症患者血清血小板生成素水平的检测及临床意义

摘　　要：目的探讨血小板减少症患者血清血小板生成素(TPO)水平的检测及其临床意义。方法血小板减少症患者176例根据不同病因分为特发性血小板减少性紫癜组(ITP组,40例)、再生障碍性贫血组(AA组,21例)、恶性血液病组(28例)、恶性肿瘤放化疗组(22例)、系统性红斑狼疮组(20例)、肝硬化脾功能亢进组(25例)和感染组(20例);另设30例健康体检者作为正常对照组。ELISA法检测各组血清TPO、Plt和巨核细胞计数,并进行相关性分析。结果与正常对照组比较,AA组、恶性血液病组及恶性肿瘤放化疗组巨核细胞计数降低(P<0.01),而TPO水平增高(P<0.01)。与正常对照组比较,ITP组巨核细胞计数增高(P<0.05)。与正常对照组比较,肝硬化脾功能亢进组巨核细胞计数增高(P<0.05),而TPO水平降低(P<0.05)。AA组、恶性血液病组和恶性肿瘤放化疗组血清TPO与Plt、巨核细胞计数呈负相关(P<0.01)。结论血清TPO水平检测可作为血小板减少症病因鉴别诊断的有用指标。Objective To investigate the detection and clinical significance of serum thrombopoietin（TPO）level in the patients with thrombocytopenia.Methods According to etiological factors,176 patients with thrombocytopenia were divided into seven groups of idiopathic thrombocytopenic purpura（group ITP,40 cases）,aplastic anemia（group AA,21 cases）,malignant hematological disease（group MHD,28 cases）,radiotherapy and chemotherapy for malignant tumors（group RC,22 cases）,systemic lupus erythematosus（group SLE,20 cases）,cirrhosis and hypersplenism（group CHS,25 cases）and infection（group I,20 cases）.Thirty healthy people were selected as normal controls（group C）.Serum TPO level and platelet（Plt）and megakaryocytes count were detected by ELISA.The correlation analysis was performed as well.Results Compared with group C,megakaryocytes count was decreased and serum TPO level was increased in groups of AA,MHD and RC（P〈0.01）.Compared with group C,megakaryocytes count was increased in group ITP（P〈0.05）.Compared with group C,megakaryocytes count was increased and serum TPO level was decreased in group CHS（P〈0.05）.Serum TPO level was negatively correlated to Plt and megakaryocytes count in groups of AA,MHD and RC（P〈0.01）.Conclusion Detection of serum TPO may be used as a useful indicator for etiologically differential diagnosis of patients with thrombocytopenia.
