摘　　要：目的:调查奥氮平门诊处方中超说明书用药情况,为促进奥氮平的合理应用提供参考。方法:抽取2016年某三甲精神专科医院奥氮平的门诊处方,从适应人群、给药剂量、适应证等方面进行分析。结果:共抽取17 073张奥氮平处方,奥氮平超说明书用药处方9319张,超说明书用药率为54.58%。其中超适应人群用药216张(1.27%),超剂量用药181张(1.94%),超适应证用药9013张(96.72%)。超适应证主要包括抑郁、阿尔茨海默病、焦虑等。结论:奥氮平门诊处方中超说明书用药普遍,具有一定的合理性和风险性,应规范其超说明书用药管理,促进奥氮平的临床合理应用。Objective： To investigate the off-label use of olanzapine prescriptions for out-patients, so as to provide reference for clinical rational application of olanzapine. Methods ： Olanzapine prescriptions for out-patients in psychiatric hospital in 2016 year were selected and analyzed according to population, dosage and indications. Results ： A total of 17 073 prescriptions were extracted, and 9 319 prescriptions were off-label used, and the off-label usage rate was 54.58%. Among them, 216（1. 27%） were used beyond adaptation population, 181（1. 94%） were overdose and 9 013（96. 72%） were beyond indications. Off-label indications mainly included depression, Alzheimer＇s disease. anxiety, etc. Conclusion： The off-label use of olanzapine prescriptions for out-patients are common, part of them are reasonable but has a risk. It is necessary to standardize off-label use and promote the rational use of olanzapine.
