新会阴保护模式对会阴保护的效果评价

摘　　要：目的评价不同会阴保护法对会阴保护的效果。方法把378例研究对象随机分为新会阴保护组和传统会阴保护组,其中新式会阴保护组290例,传统会阴保护组88例,对两种会阴保护法的保护效果进行评价,并观察会阴完整性、会阴侧切率、会阴水肿、出血量、感染、产后尿储留、新生儿情况等指标。结果新式会阴保护法会阴无裂伤率较传统会阴保护法高,Ⅱ°裂伤及Ⅲ°裂伤率较传统会阴保护法低(P<0.05),但在会阴裂伤程度上的差异无显著性(P>0.05)。新式会阴保护法会阴侧切率、会阴水肿发生率较传统会阴保护法低(P<0.05)。两种会阴保护模式产后出血量、切口或伤口感染率、产后尿潴留发生率无显著差异(P>0.05)。结论新式会阴保护法较传统会阴保护法,尽量保持了会阴的完整性,大幅降低了会阴侧切率,降低了会阴水肿的发生率。Objective To evaluate effect of different perineal protection measures on perineal protection. Methods A total of 378 patients were randomly divided into new perineal protection group（ n = 290） and traditional perineal protection group（ n = 88）,Effect of perineal protection,and the indicators such as perineum integrity,perineal incision rate,perineal edema,hemorrhage,infection,postpartum urination,and neonatal condition were evaluated. Results The new perineal protection group had higher non-perineal laceration rate than traditional perineal protection group,and lower II and III laceration rate（ P〈0. 05）,but no significant difference was seen in perineal laceration degree（ P〉0. 05）. The new perineal protection group had lower incidence of perineal incision and perineal edema than that of traditional perineal protection group（ P〈0. 05）,but there was no significant differences in the incidence rate of postpartum hemorrhage,incision or wound infection and postpartum urinary retention（ P〉0. 05）. Conclusion New perineal protection method maintains the integrity of perineum,significantly reduces the perineal incision rate and incidence of perineal edema as compared to traditional perineal protection.
