益气活血法治疗射血分数保留心力衰竭气虚血瘀证临床疗效和安全性的Meta分析

摘　　要：目的系统评价益气活血法联合西药常规治疗射血分数保留心力衰竭的临床疗效及安全性。方法检索中国学术期刊全文数据库(CNKI)及Springer图书馆中益气活血法治疗射血分数保留心力衰竭的临床随机对照试验文献,按照Cochrane协作网推荐的Jadad评分法评价入选文献的质量,采用Rev Man 5.3软件进行Meta分析。结果经筛选最终纳入22篇随机对照试验文献,均为中文文献。Meta分析结果:益气活血药联合西药可改善病人心功能(优势比=0.20,95%CI 0.15～0.24,P<0.01),改善舒张早期二尖瓣血流最大流速(E)/心房收缩期二尖瓣血流最大流速(A)比值(加权均方差=0.93,95%CI 0.48～1.38,P<0.01),增加6 min步行距离(加权均方差=0.78,95%CI 0.65～0.91,P<0.01)。结论益气活血药联合西药治疗射血分数保留心力衰竭可改善病人病情,为临床治疗射血分数保留心力衰竭提供新思路。Objective To systematically evaluate the clinical efficacy and safety of supplementing qi and activating blood therapy for heart failure with preserving ejection fraction（HFpEF）with qi deficiency and blood stasis syndrome.Methods Clinical randomized controlled trials（RCTs）of supplementing qi and activating blood therapy for the treatment of HFpEF with qi deficiency and blood stasis syndrome were retrieved from,China National Knowledge Infrastructure（CNKI）,and Springer database.The quality of the selected literatures was evaluated according to the Jadad score method recommended by the Cochrane Collaboration.Meta-analysis was performed by Rev Man 5.3 software.Results Twenty-two literatures of RCTs were enrolled,all of which were Chinese literatures.The results of meta-analysis showed that supplementing qi and activating blood therapy and western medicine could improve cardiac function（RR=0.20,95% CI 0.15～0.24,P〈 0.01）,improve E/A ratio（WMD=0.93,95% CI 0.48～1.38,P 〈0.01）,and increase the six-minute walk distance（6 MWD）（RR=0.78,95% CI 0.65～0.91,P〈 0.01）.Conclusion Supplementing qi and activating blood therapy and western medicine can improve the condition,and provide the new ideas for the clinical treatment of HFpEF with qi deficiency and blood stasis syndrome.
