尿液白细胞介素-22水平与2型糖尿病肾病病程进展的关系探讨

摘　　要：目的探讨尿液白细胞介素-22(IL-22)水平与2型糖尿病肾病病程进展之间的关系。方法选择该院内分泌病房2型糖尿病患者50例,采用酶联免疫吸附测定(ELISA)方法检测尿液中IL-22水平,同时用化学发光法检测尿清蛋白排泄率(UAER)、高效液相色谱法检测糖化血红蛋白(HbA1c)、苦味酸法检测血清肌酐用来估算肾小球滤过率(eGFR),并比较各指标的相关性。根据糖尿病肾病的诊断标准,按UAER将糖尿病患者分为3组,第1组:正常清蛋白尿组(23例);第2组:微量清蛋白尿组(15例);第3组:大量清蛋白尿组(12例)。比较各组间IL-22水平差异及组内IL-22水平与其他指标的相关性。结果 IL-22水平与UAER呈正相关,与eGFR呈负相关,差异有统计学意义(P<0.01)。按UAER分组后,各组间IL-22水平差异有统计学意义(P<0.05)。各组内IL-22与UAER呈正相关,差异有统计学意义(P<0.05);各组内IL-22与eGFR呈负相关,差异无统计学意义(P>0.05)。结论尿液中IL-22水平随UAER的增加而升高,IL-22水平与糖尿病肾病病程进展呈正相关。Objective To investigate the relationgship between the levels of interleukin 22（IL-22）in urine and the progression of type 2 diabetic nephropathy.Methods IL-22 levels in urine of 50 patients with type 2 diabetes in endocrinology ward was detected with enzyme linked immunosorbent assay（ELISA）method,and urinary albumin excretion rate（UAER）was detected chemiluminescence,glycosylated hemoglobin（HbA1 c）was detected with high performance liquid chromatographuy,serum creatinine picrate method was used to estimate glomerular filtration rate（eGFR）,and the correlation between indexes was compared.According to the diagnostic criteria of diabetic nephropathy,diabetic patients were divided into three groups according to the urinary albumin excretion rate,the first group was normal proteinuria group（n=23）,the second group was microalbuminuria group（n=15）,the third group was massive proteinuria group（n=12）.The differences between groups and the correlation between IL-22 and other indexes were compared.Results The correlation analysis of 50 patients with type 2 diabetes showed that the level of IL-22 was positively correlated with UAER,and was negatively correlated with GFR,and the difference was significant（P〈0.01）.The difference of IL-22 level between each group was significant（P〈0.05）.There was a s positive correlation between IL-22 and UAER in each group,and the difference was significant（P〈0.05）;IL-22 was negatively correlated with eGFR,and the difference was not significant（P〉0.05）.Conclusion The concentration of IL-22 in urine increases with the increase of UAER,and the level of IL-22 is positively correlated with the progression of diabetic nephropathy.
