黄芪片联合金水宝胶囊对早期糖尿病肾病患者血尿常规及sICAM-1水平、尿蛋白指标的影响

摘　　要：目的探究黄芪片联合金水宝胶囊对早期糖尿病肾病患者血尿常规及血浆可溶性细胞间黏附分子-1(sICAM-1)水平、尿蛋白指标的影响。方法选择该院自2015年3月1日至2017年3月1日收治的240例早期糖尿病肾病患者作为研究对象,按治疗方式的不同分成对照组(60例)和治疗组(180例),对照组采用饮食运动联合降糖药物治疗,治疗组又分为B组(60例)、C组(60例)、D组(60例)3组,B组采用黄芪片治疗,C组采用金水宝胶囊治疗,D组采用黄芪片联合金水宝胶囊治疗,治疗前后分别检测各组患者的尿清蛋白排泄率(UAER)、24h尿蛋白定量、sICAM-1水平、血肌酐、尿素氮、尿酸、三酰甘油、胆固醇等,每周监测血糖,治疗90d后,比较4组患者的各项检测指标变化情况。结果治疗前各组患者的24h尿蛋白定量、血肌酐、尿素氮、尿酸、三酰甘油、胆固醇、血清sICAM-1水平及UAER均无明显差异,采用不同的方式治疗后,B、C、D 3组24h尿蛋白定量、血肌酐、尿素氮、尿酸、三酰甘油、胆固醇、血清sICAM-1水平及UAER均明显低于对照组,其中以D组改善最为明显,通过对各组治疗的有效率进行计算,D组治疗总有效率最高,达96.7%,其次为C组(90.0%)、B组(86.6%)、对照组(76.7%),各组比较,差异有统计学意义(P<0.05)。结论采用黄芪片联合金水宝胶囊对早期糖尿病肾病患者进行治疗,能有效改善肾脏的血流动力学、降低其血清中尿蛋白及sICAM-I水平,从而能有效提高患者治疗的有效率。Objective To investigate the effect of Huangqi tablet combined with Jinshuibao capsule on routine hematuria,levels of plasma soluble intercellular adhesion molecule 1（sICAM-1）,and urine protein in patients with early diabetic nephropathy.Methods 240 cases of early diabetic nephropathy patients in the hospital from March 1,2015 to March 1,2017 were selected as the research subject.According to the different treatment,the patients were divided into control group（60 cases）and treatment group（180 cases）,and the treatment group was divided into B group（60 cases）,C group（60 cases）,D group（60 cases）.The control group was given the diet and exercise therapy,B group was given Huangqi tablets,C group was given with Jinshuibao capsule treatment,D group was treated with Huangqi tablet combined with Jinshuibao capsule.in the treatment of patients with urinary albumin excretion rate（UAER）,24 hours urinary protein,sICAM-1 level serum creatinine,urea nitrogen,uric acid,triglyceride,cholesterol,blood glucose were monitored weekly,90 days after treatment,changes of the indexes in 4 groups were compared.Results Before treatment,24 hours urine protein,serum creatinine,urea nitrogen,uric acid,triglyceride,cholesterol,serum levels of sICAM-1 and UAER of the patients in each group were not significantly different.After different ways of treatment,24 hours urine protein,serum creatinine,urea nitrogen,uric acid,triglyceride,cholesterol,serum levels of sICAM-1 and UAER in B,C,D groups were significantly lower than that in the control group,the D group improved the most obviously.Through the calculation of the treatment efficiency,the total effective rate in D group was96.7%,which was the highest,followed by the C group（90.0%）,B group（86.6%）and control group（76.7%）,and the differences were statistically significant（P〈0.05）.Conclusion The use of Huangqi tablets combined with Jinshuibao capsules to treat patients with early diabetic nephropathy can effectively improve the hemodynamics of the kidn
