痰脂消颗粒联合枸橼酸氯米芬治疗多囊卵巢综合征伴胰岛素抵抗45例临床研究

摘　　要：目的观察痰脂消颗粒联合枸橼酸氯米芬治疗多囊卵巢综合征伴胰岛素抵抗的临床疗效。方法选择医院2014年1月至2017年5月收治的多囊卵巢综合征伴胰岛素抵抗患者89例,随机分为对照组(44例)和治疗组(45例)。对照组患者给予枸橼酸氯米芬联合二甲双胍治疗,治疗组患者给予枸橼酸氯米芬联合痰脂消颗粒治疗,均连续治疗3个月。结果治疗组总有效率为86.67%,明显高于对照组的61.36%(χ~2=7.432,P=0.006);治疗后,两组患者促卵泡激素(FSH)大于治疗前,促黄体生成素(LH)、LH/FSH及睾酮(T)均明显小于治疗前,且治疗组改善更优(P<0.05);治疗后,两组患者胰岛素抵抗指数(IR)、胰岛素曲线下面积及糖耐量曲线下面积均明显小于治疗前,且治疗组明显小于对照组(P<0.05);治疗后,治疗组排卵率、妊娠率均明显高于对照组(P<0.05);两组治疗过程中不良反应发生率相当(6.82%比4.44%,χ~2=0.236,P=0.627)。结论痰脂消颗粒联合枸橼酸氯米芬治疗多囊卵巢综合征伴胰岛素抵抗的临床疗效显著,能明显改善内分泌代谢紊乱和胰岛素抵抗,提高排卵率和妊娠率,且安全性较好,值得推广。Objective To observe the clinical effect of Tanzhixiao Granules combined with clomiphene citrate in the treatment of polycystic ovary syndrome complicated with insulin resistance. Methods Totally 89 patients with polycystic ovary syndrome complicated with insulin resistance admitted to our hospital from January 2014 to May 2017 were selected and randomly divided into the control group（ n = 44） and the treatment group（ n = 45）. The control group was treated with clomiphene citrate combined with metformin,while the treatment group was treated with Tanzhixiao Granules combined with clomiphene citrate. The two groups were treated for3 months. Results The total effective rate of the treatment group was 86. 67%,which was significantly higher than 61. 36% of the control group（ χ~2= 7. 432,P = 0. 006）. After treatment,the levels of follicle stimulating hormone（ FSH） in the two groups were higher than those before treatment,while the levels of luteinizing hormone（ LH）,LH/FSH and testosterone（ T） in the two groups were lower than those before treatment,and the improvement of the treatment group was more significant（ P〈0. 05）. After treatment,the insulin resistance index（ IR）,the area under the curve of insulin and area under the curve of glucose tolerance of the two groups were significantly lower than those before treatment,and those in the treatment group were significantly less than those in the control group（ P〈0. 05）.After treatment,the ovulation rate and pregnancy rate of the treatment group were significantly higher than those of the control group（ P〈0. 05）. There were no significantly difference in the incidence rates of adverse reactions in the two groups during the treatment（ 6. 82% vs. 4. 44%,χ~2 = 0. 236,P = 0. 627）. Conclusion Tanzhixiao Granules combined with clomiphene citrate in the treatment of polycystic ovary syndrome complicated with insulin resistance has a good efficacy and safety,it can significantly improve the endocrine metabolic disorders,insulin
