通心络胶囊联合阿托伐他汀对老年冠心病不稳定型心绞痛患者血脂和炎性因子的影响

摘　　要：目的探讨通心络胶囊联合阿托伐他汀治疗老年冠心病不稳定型心绞痛的疗效及对血清血脂、超敏C反应蛋白(hs-CRP)和白细胞介素18(IL-18)的影响。方法选取2015年1月至2017年5月收治的老年冠心病不稳定型心绞痛患者128例,按照随机数字表法分为对照组与试验组,各64例。对照组患者给予常规治疗与阿托伐他汀治疗(20 mg/次,1次/日),试验组患者在对照组患者治疗基础上加用通心络胶囊(2粒/次,3次/日),均连续治疗2个月。结果两组患者治疗后血清总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、hs-CRP及IL-18水平均较治疗前下降(P<0.05),高密度脂蛋白胆固醇(HDL-C)较治疗前上升(P<0.05),且治疗后试验组变化更明显(P<0.05);试验组患者临床总有效率为90.63%,明显高于对照组的76.56%(P<0.05)。结论通心络胶囊联合阿托伐他汀治疗老年冠心病不稳定型心绞痛,可有效缓解临床症状,抑制炎性反应,改善血脂代谢状态。Objective To investigate the effect of Tongxinluo Capsules combined with atorvastatin on serum lipids,hs-CRP and IL-18 in elderly patients with coronary heart disease（ CHD） and unstable angina pectoris. Methods Totally 128 elderly patients with CHD and unstable angina pectoris admitted to our hospital from January 2015 to May 2017 were selected and divided into the control group and the experimental group according to the random number table method,64 cases in each group. The control group was given routine treatment and atorvastatin（ 20 mg/time,once a day）,on this basis,the experimental group was treated with Tongxinluo Cap sules（ 2 capsules/time,three times a day）,and all patients were continuously treated for 2 months. Results After treatment,the levels of serum TC,TG,LDL-C,hs-CRP and IL-18 in the two groups were significantly lower than those before treatment（ P〈0. 05）,while the levels of HDL-C in the two groups were significantly higher than those before treatment（ P〈0. 05）,and the improvement of the experimental group was more significant（ P〈0. 05）. The total effective rate of the experimental group was 90. 63%,which was significantly higher than 76. 56% of the control group（ P〈0. 05）. Conclusion Tongxinluo Capsules combined with atorvastatin in the treatment of elderly patients with CHD and unstable angina pectoris can effectively relieve the clinical symptoms,inhibit the inflammatory reaction,and improve the blood lipid metabolism status.
