摘 要:目的研究银杏叶提取物治疗慢性阻塞性肺疾病(COPD)合并肺动脉高压(PAH)的疗效和作用机制。方法将医院收治的72例COPD合并PAH患者随机分为接受常规治疗的对照组和接受银杏叶提取物+常规治疗的试验组,各36例,疗程4个月。结果治疗4个月后,两组血氧饱和度(Sp O2)、血氧分压(Pa O2)、6 min步行距离(6MWT)、第1秒用力呼气容积(FEV1)/用力肺活量(FVC)、FVC、FEV1均显著上升(P<0.05),两组平均血二氧化碳分压(Pa CO2)、肺动脉收缩压(PASP)、血浆内皮素-1(ET-1)、脑钠肽(BNP)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、丙二醛(MDA)及圣乔治呼吸问卷(SGRQ)中临床症状、疾病、活动项评分均显著下降(P<0.05),且试验组Sp O2,Pa O2,6MWT,肺功能指标,平均PASP,血浆指标及SGRQ评分变化幅度均大于对照组(P<0.05),两组间Pa CO2比较无显著差异(P>0.05);两组治疗期间均未出现头痛、胃胀、过敏等不良反应。结论银杏叶提取物具有抗脂质过氧化、清除氧自由基、降低缺氧所致内皮功能损伤和防护心肌缺血-再灌注损伤的作用,可明显改善COPD合并PAH患者的临床症状。Objective To study the effect and mechanism of Ginaton in the treatment of chronic obstructive pulmonary disease( COPD)complicated with pulmonary arterial hypertension( PAH). Methods Totally 72 patients with COPD complicated with PAH admitted to our hospital were randomly divided into the control group( the patients were given the conventional therapy) and the experimental group( the patients were given Ginaton combined with conventional therapy), 36 cases in each group. The course of treatment was 4 months.Results After 4 months of treatment,the blood oxygen saturation( Sp O2),partial pressure of oxygen( Pa O2),6 min walking distance( 6 MWT),forced expiratory volume in 1 second( FEV1)/forced vital capacity( FVC),FVC and FEV1 in the two groups were significantly increased( P〈0. 05),while the partial pressure of carbon dioxide( Pa CO2),pulmonary artery systolic pressure(PASP),plasma endothelin-1( ET-1),brain natrinretic peptide( BNP),creatine kinase( CK),creatine kinase isoenzyme( CK-MB),malondialdehyde( MDA),scores of clinical symptoms,diseases and activities in St Georges Respiratory Questionnaire( SGRQ) in the two groups were significantly decreased( P〈0. 05),and the improvement of Sp O2,Pa O2,6 MWT,lung function indexes,the mean PASP,plasma indexes and SGRQ scores in the experimental group was more significant than that in the control group( P〈0. 05). There was no significant difference in Pa CO2 between the two groups( P〈0. 05). There was no headache,bloating,allergy and other adverse reactions during treatment in the two groups. Conclusion Ginaton has the effects of anti-lipid peroxidation,scavenging oxygen free radicals,reducing hypoxia-induced endothelial dysfunction and protecting myocardial ischemia-reperfusion injury,it can significantly improve the clinical symptoms of patients with COPD complicated with PAH.
