绵阳市基层医疗机构基本药物制度执行状况调查与分析

摘　　要：目的调查绵阳市基本药物制度实施后基层医疗机构政策落实及收支变化情况。方法抽样调查四川省绵阳市46家基层医疗机构,收集基本药物制度实施后各抽样机构执行情况及收支结构等数据,并进行统计与分析。结果实施基本药物制度后,基层医疗机构药品配置使用以基本药物为主,占比在99%以上;基层医疗机构业务量呈上升趋势,财政补助收支结余增加,但业务收支亏损明显,总收支结余连年下降。结论基本药物制度在绵阳地区基层医疗机构落实较好;实施基本药物制度后,基层医疗机构业务呈良好发展态势,但亏损仍长期存在;政府投入对基层医疗机构发展作用明显,需建立及完善激励机制及长效补偿机制。Objective To investigate the implementation of the policy and the financial condition in grassroots medical institutions after the implementation of national essential drug system（ NEDS） in Mianyang city. Methods A total of 46 hospitals were investigated by sample survey. After the implementation of the NEDS,the data of the implementation of the policy and the structure of revenue and expenditure in all the institutions were collected and analyzed. Results After the implementation of NEDS,the essential drug accounted for more than 99% of the whole drug system in the grassroots medical institutions. The workload of the grassroots medical institutions was on the rise,and the balance of revenue and expenditure of financial subsidies had increased. However,the deficit of business revenue and expenditure was obvious,and the balance of total revenue and expenditure had declined year after year. Conclusion The NEDS is well implemented in the grassroots medical institutions in Mianyang city. After the implementation of NEDS,the workload of the grassroots medical institutions has shown a good development trend,but financial loss still exists for a long time. Government assistance plays a significant role in the development of grassroots medical institutions. It is necessary to establish and improve the incentive mechanism and long-term compensation mechanism.
