鼻内镜微创手术联合曲安奈德鼻喷雾剂及鼻腔冲洗治疗鼻息肉并发鼻窦炎的疗效分析

摘　　要：目的分析鼻内镜微创手术联合曲安奈德鼻喷雾剂及鼻腔冲洗治疗鼻息肉并发鼻窦炎的临床疗效。方法将2017年9月—2018年1月我院治疗的60例鼻息肉并发鼻窦炎患者作为研究对象,将其依照随机数字表法分为2组各30例。对照组采取常规药物治疗,于对照组基础上观察组加用鼻内镜微创手术、曲安奈德鼻喷雾剂、鼻腔冲洗等治疗。观察2组临床疗效、治疗前后视觉模拟量表(VAS)评分、Lund-Mackey鼻窦CT评分、气道阻力、嗅觉CCCRC评分与不良反应发生情况。结果观察组总有效率高于对照组,差异有统计学意义(P<0.05);治疗前2组VAS评分、鼻窦CT评分、气道阻力、嗅觉CCCRC评分对比差异无统计学意义(P>0.05);治疗后观察组VAS评分、鼻窦CT评分、气道阻力低于对照组,嗅觉CCCRC评分高于对照组,差异有统计学意义(P<0.05);2组不良反应发生率差异无统计学意义(P>0.05)。结论鼻内镜微创手术联合曲安奈德鼻喷雾剂及鼻腔冲洗治疗鼻息肉并发鼻窦炎疗效确切且安全性较高,可改善气道阻力,促进嗅觉功能恢复与疾病转归。bObjective To analyze the clinical efficacy of nasal endoscopic minimally invasive surgery combined with triamcinolone acetonide nasal spray and nasal irrigation in treatment of nasal polyp with nasosinusitis. Methods 60 patients with nasal polyp plus nasosinusitis treated in our hospital from September 2017 to January 2018 were selected, and they were divided into two groups by random number table method, with 30 cases in each group.Control group was given routine medicine, while observation group was treated with nasal endoscopic minimally invasive surgery, triamcinolone acetonide nasal spray and nasal irrigation based on control group. The clinical efficacy, visual analogue scale（VAS） score,Lund-Mackey paranasal sinus CT score, airway resistance, olfactory CCCRC score before and after treatment,and adverse reactions in two groups were observed. Results The total effective rate of observation group was higher than that of control group（P〈0.05）.Before treatment, there was no statistical difference in the VAS score, paranasal sinus CT score, airway resistance, olfactory CCCRC score between the two groups（P〈0.05）.After treatment,the VAS score, paranasal sinus CT score, airway resistance in observation group were lower than those in control group, while the olfactory CCCRC score was higher than control group（P〈0.05）.There was no statistical difference in the incidence of adverse reactions between the two groups（P〈0.05）.Conclusion Nasal endoscopic minimally invasive surgery combined with triamcinolone acetonide nasal spray and nasal irrigation in treatment of nasal polyp with nasosinusitis is effective and safe, it can improve airway resistance, promote olfactory recovery and prognosis of disease.
