单侧多功能外固定支架对开放性胫腓骨骨折患者骨代谢及创伤程度的影响

摘　　要：目的研究单侧多功能外固定支架对开放性胫腓骨骨折患者骨代谢及创伤程度的影响。方法选择2014年1月至2017年2月我院收治的104例开放性胫腓骨骨折患者,根据术式不同分为对照组和研究组,各52例。对照组予以钢板内固定治疗,研究组采用单侧多功能外固定支架治疗,比较两组患者的临床疗效、并发症发生情况、骨代谢及创伤程度。结果研究组的治疗总有效率明显高于对照组(P<0.05);研究组的术后并发症总发生率明显低于对照组(P<0.05);术后7 d,两组骨代谢指标BGP、P1NP、BALP水平均升高,β-CTX水平及创伤程度指标Hp、CRP、CER水平均降低,且研究组均优于对照组(P<0.05)。结论单侧多功能外固定支架治疗开放性胫腓骨骨折疗效显著,可有效改善创伤及感染情况,骨折端愈合良好,安全可靠,值得临床推广。Objective To study the effect of unilateral multifunctional external fixator on bone metabolism and injury severity in patients with open tibiofibula fractures. Methods A total of 104 patients with open tibiofibula fractures in our hospital from January 2014 to February 2017 were selected and divided into control group and study group according to the different surgical treatment, with 52 cases in each group. The control group was treated with plate internal fixation and the study group was treated with unilateral multifunctional external fixator. The clinical effects, incidences of postoperative complications, bone metabolism and injury severity of the two groups were compared. Results The total effective rate of treatment and incidence of postoperative complications of the study group were significantly better than those of the control group（P0.05）. At postoperative seven days, the levels of BGP, P1 NP and BALP of the two groups significantly increased, while the levels of β-CTX, Hp, CRP and CER of the two groups significantly decreased, and those indexes of the study group were better than the control group（P 0.05）. Conclusion Unilateral multifunctional external fixator is effective in the treatment of open tibiofibula fractures, which can effectively improve injury severity and infection, and it has a good fracture healing. It is safe and reliable and worthy of clinical application.
