云南省曲靖市门诊女性人群HPV分型检测结果分析

摘　　要：目的研究云南省曲靖市女性门诊病人HPV感染状况及其基因型分布情况。方法回顾性分析2014年1月1日至2016年11月30日在曲靖市第一人民医院就诊的女性5 510例,将筛查人群分为≤25岁、26~35岁、36~45岁、46~55岁和≥56岁5个年龄组。统计分析每个年龄组HPV的感染率及27种亚型的检出情况。结果在5 510例门诊女性病人中,HPV阳性率为28.49%(1 570/5 510)。单一感染率为75.03%,二重、三重和四重及以上感染率分别为18.73%、4.78%和1.47%;在单一感染者中,hr HPV占70.20%,最常见的hr HPV是HPV16、HPV52、HPV58、HPV18,感染率分别为18.51%、10.02%、8.40%和4.67%;lr HPV占29.79%,最常见的lr HPV是HPV6、HPV11,感染率分别为11.97%和6.79%。每个年龄组的感染率分别为38.85%、27.11%、23.45%、28.04%和47.29%,其差异具有统计学意义(P<0.001)。结论云南省曲靖地区HPV感染率高,最常见高危型是HPV16、HPV52、HPV58、HPV18。对HPV基因亚型分析是临床宫颈病变诊断、早期筛查的理想方法,建议将HPV52、58纳入HPV疫苗筛选评估。曲靖地区HPV感染率较高的是≤25岁和≥56人群,应该加强对该人群的检测和预防。Objective To investigate the prevalence and genotype distribution of HPV infection in female outpatients in Qujing, Yunnan. Methods A retrospective analysis of 5 510 women in Qujing First Peopie＇s Hospital from January 1,2014 to November 30,2016 was carried out. The screening population was divided into 5 age groups of ≤ 25,26 - 35,36 -45,46 ~ 55 and ≥56 years old. The infection rate of HPV in each age group and the detection of 27 subtypes were analyzed statistically. Results Among 5510 cases of female outpatient,the positive rate of HPV accounted for 28. 49% （ 1 570/5 510）. The single infection rate occupied 75.03% ,and the infection rates of double,triple,quadruple and above held 18. 73% ,4. 78% and 1.47%. Among the single infected cases,hrHPV accounted for 70. 20% ,and the most common hrHPV were HPV16,HPV52, HPV58 and HPV18, and the infection rates were 18.51%, 10. 02% ,8.40% and 4. 67% respectively;lrHPV accounted for 29.79% ,and the most common lrHPV were HPV6 and HPVll. The rates of infection were 11.97% and 6. 79% , respectively. The infection rate in each age group were 38. 85%, 27. 11%, 23.45% , 28.04% and 47. 29%, respectively. The difference was statistically significant （ P 〈 0. 05 ）. Conclusion HPV infection rate is high in Qujing area of Yunnan Province, and the most common high - risk types are HPV16, HPV52, HPV58 and HPV18. HPV gene subtype analysis is an ideal method for the diagnosis and early screening of clinical cervical lesions. It is suggested that HPV52 and 58 should be included in HPV vaccine screening and evaluation. A higher rate of HPV infection in Qujing is less than 25 and more than 56 people, the detection and prevention of the crowd should be strengthened.
