艾滋病相关耳鼻咽喉头颈部卡波西肉瘤六例临床分析

摘　　要：目的提高耳鼻喉科医师对耳鼻咽喉头颈部艾滋病相关卡波西肉瘤(KS)的临床诊疗能力。方法回顾性分析2011年1月至2017年1月年首都医科大学附属北京佑安医院收治的6例艾滋病耳鼻咽喉头颈部KS患者的临床资料,对其临床表现、治疗及随访结果进行分析、总结。结果肿瘤位于鼻部1例,咽部3例,喉部2例,表现为局部暗褐色结节样或斑片样新生物,行手术治疗1例,术后给予抗病毒治疗及化疗,余5例行抗病毒治疗及化疗,随访1年有1例死亡。结论 KS患者首诊于耳鼻咽喉头颈外科,可根据其专科临床表现及是否有艾滋病全身表现全面了解病史,以期早期诊断,及时治疗。Objective To improve the clinical diagnosis and treatment ability of otolaryngologist on AIDS-related Kaposi’s sarcoma（KS） in Department of Otolaryngology-Head and Neck. Methods The clinical data of 6 patients with AIDS-related KS in otolaryngology-head and neck treated in Beijing Youan Hospital, Capital Medical University were retrospectively analyzed from January 2011 to January 2017. The clinical manifestations, treatment and follow-up results were analyzed and summarized. Results The tumor was located in nose in 1 case, pharynx in 3 cases, larynx in 2 cases, with clinical manifestation of local dark brown nodular or patchy neoplasm. One patient was treated with surgery and received antiviral therapy and chemotherapy after operation, and the remaining 5 patients were treated with antiviral therapy and chemotherapy. One case died after follow-up for one year. Conclusion KS patients are first diagnosed in Department of Otolaryngology-Head and Neck, and the medical history could be fully understood according to its clinical manifestations and the whole body performance of AIDS, with a view for early diagnosis and timely treatment.
