摘 要:目的分析血清前白蛋白(PA)、B型脑钠肽(BNP)、C-反应蛋白(CRP)及降钙素原(PCT)联合检测在小儿急性呼吸道感染早期诊断中的价值。方法收集我院自2015年6月-2017年7月期间收治临床确诊的急性呼吸道感染患儿200例,其中细菌感染组93例,非细菌感染组107例,另收集同期于我院行健康体检的儿童90例作为对照组,分别测定三组的PA、BNP、CRP及PCT水平,同时测定并分析感染组治疗前后的PA、BNP、CRP及PCT水平。结果细菌感染组血清BNP、CRP及PCT水平均分别明显高于非细菌感染组与对照组(P<0.05);PA水平均明显低于非细菌感染组与对照组(P<0.05);而非细菌感染组的PA、BNP、CRP及PCT水平与对照组对比差异无统计学意义(P>0.05)。细菌感染组血清PA、BNP、CRP、PCT水平阳性率及其联合检测阳性率均分别明显高于非细菌感染组与对照组(P<0.05);虽然非细菌感染组的各项指标阳性率略高于对照组,但两组相比差异无统计学意义(P>0.05)。细菌感染组在给予抗感染治疗3d之后,症状及体征均明显好转,且血清PA水平显著高于治疗前(P<0.05),BNP、CRP及PCT水平明显低于治疗前(P<0.05);CRP水平基本恢复至正常水平。结论 PA、BNP、CRP及PCT联合检测对鉴别及诊断小儿早期急性呼吸道感染具有理想的价值,同时可指导临床治疗,并为预后判断提供参考。Objective To analyze the value of serum prealbumin( PA),B-type brain natriuretic peptide( BNP),C-reactive protein( CRP) and procalcitonin( PCT) in the early diagnosis of acute respiratory tract infection in children. Methods A total of 200 patients with acute respiratory tract infection were enrolled in our hospital from June 2015 to July 2017,including 93 cases of bacterial infection group and 107 cases of non-bacterial infection group. 90 cases of our hospital health examination of children were taken as the control group. The levels of PA,BNP,CRP and PCT were measured in the three groups,and the levels of PA,BNP,CRP and PCT were measured before and after treatment in the bacterial infection group. Results The levels of serum BNP,CRP and PCT in the bacterial infection group were significantly higher than those in the non-bacterial infection group and the control group( P〈0. 05),and PA level was significantly lower than that in the non-bacterial infection group and the control group( P〈0. 05). There was no significant difference in PA,BNP,CRP and PCT between the non-bacterial infection group and the control group( P〈0. 05). The positive rate of serum PA,BNP,CRP,PCT and the positive rate of combined detection were significantly higher in the bacterial infection group than in the non-bacterial infection group and the control group( P〈0. 05). Although the positive rate of the non-bacterial infection group was slightly higher than in the control group,the difference between the two groups was not statistically significant( P〈0. 05). After treatment with anti-infective therapy,the symptoms and signs of bacterial infection were significantly improved,and serum PA level was significantly higher than before( P〈0. 05). BNP,CRP and PCT were significantly lower than those before treatment( P〈0. 05). CRP level returned to normal level.
