摘　　要：目的应用超声心动图评价阻塞性睡眠呼吸暂停综合征(OSAS)对左心室舒张功能的影响。方法选择呼吸科睡眠呼吸暂停综合征患者90例,患者均经多导睡眠仪监测7 h以上确诊。依据患者的呼吸暂停低通气指数(AHI),根据病情程度将分为3组:AHI 5~20次/h为轻度OSAS组、AHI 21~40次/h为中度OSAS组,AHI≥40次/h为重度OSAS组,每组30例,选择30例健康体检者为对照组。结果重度OSAS组、中度OSAS组、轻度OSAS组与对照组年龄、体重、性别比、血压差异无统计学意义(P>0.05),AHI在不同组间差异有统计学意义(P<0.05);重度OSAS组、中度OSAS组、轻度OSAS组与对照组间LVDD、LVSD、LPWT、IVST的测量值随着OSAS患者病情的加重而增大,但差异无统计学意义(P>0.05);重度OSAS组、中度OSAS组、轻度OSAS组与对照组4组间两两比较,4组DT延长、E峰减低、A峰增加、E/A减低,E峰、E/A在病例组及对照组间差异有统计学意义(P<0.05),A峰、DT值组间统计学差异无统计学意义(P>0.05);轻度OSAS组、中度OSAS组、重度OSAS组分别与对照组两两比较,OSAS患者病情越重,E’越低,E’/A’比值越低,E/E’比值越大,4组比较差异有统计学意义(P<0.05)。结论超声心动图可检测出OSAS患者左心室肥厚、左心室舒张功能减低且与OSAS病情程度相关联。Objective To evaluate the effects of obstructive sleep apnea-hypopnea syndrome（ OSAS） on left ventricle diastole function of patients by ultrasonic cardiogram. Methods A total of 90 patients with OSAS who were diagnosed by polysomnography monitoring over 7 hours and treated in our hospital from March 2015 to March 2016 were enrolled in the study. According to the severity of sleep apnea hypopnea index（ AHI）,these patients were divided into 3 groups： mild OSAS group（ 5/h 〈 AHI 〈 20/h）,moderate OSAS group（ 20/h 〈 AHI 〈 40/h） and severe OSAS group（ AHI≥40/h）,with 30 patients in each group. At the same time,the other 30 healthy subjects who received physical examination in our hospital were enrolled as control group. Results There were no significant differences in patient; s age,body mass,sex ratio and blood pressure among the four groups（ P 〉 0. 05）. However there was significant difference in AHl among the four groups（ P 〈 0. 05）. The levels of LVDD,LVSD,LPWT and IVST were increased along with the increase of severity of OSAS,however there were no significant differences among the four groups（ P 〉 0. 05）. There were significant differences in E peak and E/A between case groups and control group（ P 〈 0. 05）. however there were no significant differences in A peak and DT value by interblock comparison among groups（ P 〉 0. 05）. As compared with those in control group,the more severity of OSAS,the lower E’,the lower E’/A’,the higher E/E’in the three case groups（ P 〈 0. 05）. There were significantly differences in E’,E’/A’,E/E’among the four groups（ P 〈 0. 05）. Conclusion The ultrasound cardiography can detect left ventricular hypertrophy and left ventricular diastolic function decrease in patients with OSAS,moreover,which are correlated with the severity of OSAS.
