血浆D-二聚体含量在急性主动脉夹层与非ST段抬高型急性冠脉综合征鉴别中的意义

摘　　要：目的:探索血浆D-二聚体含量在急性主动脉夹层与非ST段抬高型心肌梗死患者中的差异,及其在二者鉴别中的意义。方法:回顾性分析就诊于我院胸痛中心的53例急性主动脉夹层和104例非ST段抬高型急性冠脉综合征患者,比较两组患者血浆D-二聚体含量,通过ROC曲线分析血浆D-二聚体含量对急性主动脉夹层的鉴别意义。结果:急性主动脉夹层患者血浆D-二聚体含量明显高于非ST段抬高型急性冠脉综合征患者(P<0.05)。ROC曲线分析显示血浆D-二聚体含量在该人群中对主动脉夹层有鉴别意义(P<0.05)。结论:急性主动脉夹层患者血浆D-二聚体含量明显高于非ST段抬高型急性冠脉综合征患者,急诊血浆D-二聚体含量可用于二者的鉴别。Objective： To investigate the difference of plasma D-dimer levels between patients with acute aortic dissection（ AAD） and ones with non ST-segment elevated acute coronary syndrome（ NSTACS）,and the value of D-dimer was assessed in the differential diagnosis. Methods： This retrospective study collected data from 53 patients with AAD and 104 patients with NST-ACS in the Chest Pain Center of the Affiliated People＇s Hospital of Jiangsu University. Then the difference of plasma D-dimer was compared between patients with AAD and NST-ACS. ROC curve was used to investigate the value of plasma D-dimer in the diagnosis of AAD. Results： Patients with AAD had higher levels of plasma D-dimer than patients with NST-ACS（ P〈0. 05）. ROC curve showed plasma D-dimer level could be used to differentiate AAD from NST-ACS（ P〈0. 05）. Conclusion： Patients with AAD had higher levels of plasma D-dimer than patients with NST-ACS,and the D-dimer levels could be used to differentiate AAD from NST-ACS.
