摘 要:目的研究逆向静脉穿刺在掌指关节、腕关节附近血管穿刺中的临床应用效果。方法选择2016年1月至2017年11月在我院消化神经内科进行静脉穿刺治疗的300例患者,随机分为观察组与对照组,各150例。对照组患者实施常规顺向静脉穿刺法,观察组实施逆向静脉穿刺法。比较两组患者的一次采血足量率、渗漏率及一次穿刺成功率,并观察两组患者穿刺时的疼痛程度。结果观察组患者的一次采血足量率及一次穿刺成功率均明显高于对照组(P<0.05),观察组的渗漏率明显低于对照组(P<0.05);观察组0~1级穿刺疼痛率为64.00%(96/150),明显高于对照组的42.00%(63/150)(P<0.05)。结论与常规的顺向静脉穿刺法相比,掌指关节、腕关节附近采用逆向静脉穿刺法可以有效降低采血不足发生率及渗漏率,提高采血足量发生率以及一次穿刺成功率,并可显著降低患者的穿刺疼痛感,值得临床推广应用。Objective To study the clinical effect of reverse vein puncture in the blood vessel puncture near the knuckle and wrist joint. Methods From January 2016 to November 2017, 300 patients in digestive neurology department of our hospital were randomly divided into observation group and control group, with 150 cases in each group. The patients in the control group were given conventional anterograde venipuncture, while the observation group was given retrograde venipuncture. The incidences of sufficient blood collection, the leakage rate and the success rate of one puncture were compared between the two groups, and the degree of pain in the two groups were observed. Results The incidence of adequate blood collection and the success rate of one puncture in the observation group were significantly higher than those in the control group(P〈0.05). The incidence of the leakage rate of the observation group were significantly lower than those of the control group(P〈0.05). The incidence of pain at grade 0 and 1 in the observation group was 64.00%(96/150), which was significantly higher than 42.00%(63/150) of the control group, the difference was stastisticly significant(P〈0.05). Conclusion Compared with the conventional anterograde venipuncture method, retrograde venipuncture near the knuckle and wrist joint can effectively reduce the incidence of insufficient blood collection and blood leakage rate, improve the incidence of adequate blood collection and the success rate of one puncture, and can significantly reduce the puncture pain, which is worthy of clinical application.
