Invasive and Non-invasive Ventilator Sequential Therapy for Chronic Obstructive Pulmonary Disease Complicated with Respiratory Failure

摘  要:目的探讨有创及无创呼吸机序贯治疗慢性阻塞性肺疾病(COPD)合并呼吸衰竭的临床疗效。方法选取2016年1—12月该院收治的100例COPD合并呼吸衰竭的患者,随机双盲分为对照组(n=50)和观察组(n=50),对照组采用常规有创治疗,观察组采用有创-无创序贯治疗,比较两组的治疗疗效,相关肺炎(VAP)的发生率及住院时间。结果结果显示,观察组患者治疗总有效率为96.00%,显著高于对照组76.00%,差异有统计学意义(χ~2=12.095,P=0.001 7<0.05);观察组患者VAP发生率、住院时间分别为1例(2.00%)、(15.32±2.78)d,显著优于对照组8例(16.00%)、(18.01±2.90)d,差异有统计学意义(χ~2=8.016,t=12.443;P=0.002 9,0.001 8<0.05)。结论有创及无创呼吸机序贯治疗慢性阻塞性肺部疾病合并呼吸衰竭,能显著提高治疗有效率,降低并发症的发生,并减少住院时间。Objective This paper tries to investigate the clinical efficacy of invasive and noninvasive ventilator in the treatment of chronic obstructive pulmonary disease(COPD) with respiratory failure. Methods From January 2016 to December 2016, 100 COPD patients with respiratory failure treated in our hospital were randomly divided into a control group(n=50) and an observation group(n=50). The control group received conventional invasive treatment. The observation group used invasive-non-invasive sequential therapy to compare the therapeutic efficacy of the two groups, the incidence of associated pneumonia(VAP) and length of stay. Results The results showed that the total effective rate of observation group patients was 96.00%, which was significantly higher than that of the control group 76.00%. The difference was significant(χ~2=12.095, P=0.001 7〈0.05). The incidence of VAP and length of stay in the observation group were 1 cases(2.00),(15.32±2.78)d, significantly better than the control group 8 cases(16.00),(18.01±2.90)d, the difference was significant,(χ~2=8.016, t=12.443; P=0.002 9, 0.001 8〈0.05). Conclusion Invasive and non-invasive ventilator sequential treatment of chronic obstructive pulmonary disease combined with respiratory failure can significantly increase treatment efficiency, reduce complications, and reduce hospital stay.
