超声引导下骶髂关节腔内注射治疗骶髂关节炎的临床疗效研究

摘　　要：目的分析超声引导下骶髂关节腔内注射治疗骶髂关节炎的临床疗效、成功率及术后并发症发生情况。方法选取四川省医学科学院于2013年10月至2016年10月收治的骶髂关节炎患者80例,按照随机数字表法将患者分为超声引导下骶髂关节腔内注射治疗组(超声引导组,n=40)和CT引导下骶髂关节腔内注射治疗组(CT引导组,n=40),对两组患者的疾病活动指数(BASDAI)、功能指数(BASFI)、视觉模拟评分(VAS评分)、临床疗效、穿刺成功情况及术后并发症发生情况进行统计分析。结果两组患者治疗后BASDAI、BASFI、VAS评分均明显低于治疗前,差异有统计学意义(P<0.05);治疗前后两组患者BASDAI、BASFI、VAS评分比较,差异无统计学意义(P<0.05);超声引导组治疗总有效率82.5%(33/40)、穿刺成功率85.0%(34/40)和CT引导组治疗总有效率90.0%(36/40)、穿刺成功率95.0%(38/40)比较,差异均无统计学意义(P>0.05);两组患者术后并发症发生率均为10.0%(4/40)。结论超声和CT引导下骶髂关节腔内注射治疗骶髂关节炎均安全有效,相比而言,超声引导下骶髂关节腔内注射治疗骶髂关节炎更加简便、经济且辐射小。Objective To analyze the clinical efficacy, success rate and postoperative complications of ultrasound-guided intra-articular injection of sacroiliac joint in the treatment of sacroiliac arthritis. Methods A total of 80 cases with sacroiliac arthritis treated in Sichuan Academy of Medical Sciences from October of 2013 to October of 2016 were selected and divided randomly into the ultrasound-guided intra-articular injection group （ultrasound-guided group, n =40） and CT-guided intra-articular injection group （CT-guided group, n =40）. The Bath Ankylosing Spondylitis disease activity index （BASDAI）, Bath Ankylosing Spondylitis function index （BASFI）, visual analogue score （VAS）, clinical efficacy, puncture success and postoperative complications were statistically analyzed and compared between both groups. Results The scores of BASDAI, BASFI and VAS after treatment were significantly lower than before treatment in both groups （ P 〈0.05）. The two groups were not significantly different in the scores of BASDAI, BASFI and VAS before and after treatment. The total effective rate and success rate of puncture were 82.5% （33/40） and 85.0% （34/40） respectively in the ultrasound-guided group and they were 90.0% （36/40） and 95.0% （38/40） respectively in the CT-guided group, and there were no significant differences between the two groups （ P 〉0.05）. The incidences of postoperative complications were 10.0% （4/40） in both groups. Conclusion Uitrasound and CT guidance of the sacroiliac joint cavity injection for the treatment of sacroiliac arthritis is safe and effecitive. In contrast, ultrasound guidance is more convenient, economical and less radiative.
