摘 要:目的探究血液灌流技术治疗急性重度杀虫双中毒的疗效分析。方法方便选择该院自2014年8月—2016年8月间收治的急性重度杀虫双中毒的患者104例,随机分为对照组和研究组,对照组52例采用常规治疗,研究组患者在常规治疗基础上加血液灌流治疗,观察两组患者的临床各项指标、治疗前后的血中农药浓度以及并发症情况。结果研究组患者治愈率为94.23%,远高于对照组的82.69%,(χ~2=3.433,P<0.05);研究组24 h后血中农药浓度仅为(0.53±0.14)μmol/L,显著低于对照组(0.93±0.23)μmol/L,两者差异有统计学意义(t=10.713,P<0.05);研究组患者的并发症发生率为13.46%,对照组患者的并发症的发生率为69.23%,两者差异有统计学意义(χ~2=17.632,P=0.000)。结论临床急诊上对于治疗急性重度杀虫双中毒的患者,应用血液灌流会取得理想的治疗效果,可以显著降低患者血液中的农药浓度并且降低东莨菪碱使用量,在一定程度上减低了治疗中并发症的发生率,值得在临床推广使用。Objective To study the curative effect of hemoperfusion in treatment of acute severe ditoxicosis. Methods 104 cases of patients with acute severe ditoxicosis admitted and treated in our hospital from August 2014 to August 2016 were convenient selected and randomly divided into two groups with 52 cases in each, the control group used the routine therapy,while the research group used the hemoperfusion on the basis of the control group, and various clinical indexes, pesticide concentrations in blood and complications of the two groups were observed. Results The clinical cure rate in the research group was much higher than that in the control group(94.23% vs 82.69%,χ~2=3.433,P0.05), and the pesticide concentration in blood after 24 h in the research group was obviously lower than that in the control group[(0.53±0.14)μmol/L vs(0.93±0.23)μmol/L], and the difference was obvious(t =10.713,P 0.05), and the incidence rate of complications in the research group and in the control group was respectively 13.46% and 69.23%, and the difference was obvious, with statistical significance(χ~2=17.632, P=0.000). Conclusion The curative effect of hemoperfusion in emergency treatment of acute severe ditoxicosis is obvious, which can obviously reduce the pesticide concentrations in blood and reduce the usage of scopolamine, and it can reduce the incidence rate of complications to a certain degree, and it is worth clinical promotion and application.
