摘  要:目的 :比较低温等离子射频消融术及CO_2激光治疗早期喉癌及癌前病变的效果与安全性,为早期喉癌及癌前病变治疗方法的选择提供参考依据。方法 :纳入观察的184例早期喉癌及癌前病变患者将我院于2014年3月至2017年1月于我院接受低温等离子射频消融术(射频组,92例)及CO_2激光手术(激光组,92例)。比较两组患者手术情况、恢复情况及并发症发生情况,分析两种术式的近期治疗效果与安全性。结果 :射频组手术时间低于激光组,差异有统计学意义(P<0.05),两组术中出血量、术后VAS评分比较,差异无统计学意义(P>0.05)。射频组术后1个月黏膜恢复情况评分、基频微扰、振幅微扰低于激光组,其谐噪比高于后者,差异有统计学意义(P<0.05)。两组患者均未见术后并发症发生;射频组随访期间未见复发,激光组1例术前T1b患者复发,行喉全切除术治疗。结论 :低温等离子射频消融术及CO_2激光治疗早期喉癌及癌前病变的效果与安全性均值得肯定,与CO_2激光相比,低温等离子射频消融术在黏膜保护、近期声音恢复方面的优势更为明显。Objective: The objective of this study was to compare the effects and safety of low-temperature plasma radiofrequency ablation and CO2 laser surgery in the treatment of early laryngeal carcinoma and precancerous lesions, and to provide references for the selection of treatment methods for early laryngeal carcinoma and precancerous lesions. Methods: A total of 184 patients with early laryngeal carcinoma and precancerous lesions received low-temperature plasma radiofrequency ablation (radiofrequency group, n=92) and CO2 laser surgery (laser group, n=92) in our hospital between March 2014 and January 2017 were observed. The operation, recovery and complications of the two groups were compared, and the recent therapeutic effects and safety of the two methods were analyzed. Results: The operation time of the radiofrequency group was lower than that of the laser group, and the difference was statistically signifcant (P〈0.05). There was no statistically signifcant difference between the two groups in the intraoperative bleeding volume and the postoperative VAS score (P〉0.05). At 1 month after the operation, the mucosal recovery score, the fundamental frequency perturbation, and the amplitude perturbation in the radiofrequency group were lower than those in the laser group, and the harmonic to noise ratio in the radiofrequency group was higher than the latter, and the difference was statistically signifcant (P〈0.05). No postoperative complications occurred in the two groups. No recurrence occurred in the radiofrequency group during the follow-up period. Only 1 preoperative T1b patient in the laser group recurred and was treated with total laryngectomy. Conclusions: The effects and safety of low-temperature plasma radiofrequency ablation and CO2 laser surgery in the treatment of early laryngeal carcinoma and precancerous lesions are worthy of affrmation.
