摘 要:目的检测浆细胞样树突状细胞(pDC)及Toll样受体7、9(TLR7,TLR9)在鲍温样丘疹病(BP)皮损中的分布及表达情况,探讨其在BP发病机制中的作用。方法采用免疫组化SP法检测36例BP患者皮损中CD123^+pDCs及TLR7、TLR9的表达情况,以皮损旁正常皮肤组织作为对照。并对CD123^+pDCs密度与TLR7、TLR9表达水平进行相关性分析。结果CD123^+pDCs、TLR7、TLR9阳性细胞主要位于真皮乳头层。与皮损旁正常皮肤组织比较,BP皮损中CD123+pDCs密度(0.351±0107)增高,胞体较大呈多角形。TLR7和TLR9在皮损真皮中的阳性率半定量评分[分别为(5083±2.285)和(4.639±2.072)]均比在皮损旁正常皮肤组织真皮和表皮中的阳性率[(3.611±1.498)和(3.278±1.137)]升高。真皮乳头层CD123^+pDCs密度与TLIk7、TLR9的表达强度在皮损组织及皮损旁正常皮肤组织均呈直线正相关(P〈0.05)。结论BP皮损真皮组织中CD123^+pDCs密度增高,TLR7、TLR9表达增强,提示HPV感染时三者有一定程度的激活,推测这种激活程度可能并不足以清除临床病毒感染而使BP皮损持续存在,这可能与BP的发生发展密切相关。Objective To determine the expression of cluster of differentiation 123, an antigen marker of plasmacytoid dendritic cells, positive cells ( CD123+pDCs ) and toll-like receptor 7 and 9 ( TLR7 and TLR9 ) in bowenoid papulosis ( BP ) lesions and adjacent normal skin tissue, and to explore the role of them in the pathogenesis of BP. Methods In our research, the immunohistochemical staining was used to determine the distribution and density of CD 123+pDCs and the expression of its related functional molecules TLR7 and TLR9 in BP lesions from 36 patients, using corresponding adjacent normal skin tissue as a negative control. Correlation analyses of CD123+pDCs density and TLR7, TLR9 expression were performed. Results CD123+pDCs were expressed mainly in monocytes in the papillary dermis. TLR7 and TLR9 were expressed in the cytoplasm of monocytes in the upper dermis. In the papillary dermis of BP lesions, CD123+pDCs were larger with a polygonal shape, and its density was higher than in the adjacent normal papillary dermal tissue. The semi-quantitative scores of TLR7 and TLR9 expression were significantly corresponding higher in the dermis of BP lesions than in the adjacent normal dermis. Pearson correlation analysis showed a positive linear relationship between CD123+pDCs density and TLR7 or TLR9 expression in the papillary dermis of BP lesions, and in the adjacent normal papillary dermis. Conclusion In the dermis of BP lesions, higher CD123+pDCs density and higher expression of TLR7 and TLR9 suggest that after HPV infection, all of them have a degree of immune activation. However, the degree of activation of them is insufficient to eradicate HPV infection probably, leading to persistent BP lesions. This change may be closely related to the development and progression of BP.
