肠内营养制剂联合维生素D对老年废用性肌萎缩症患者肌肉力量及骨折事件的影响

摘　　要：目的探讨肠内营养制剂联合维生素D对老年废用性肌萎缩症患者肌肉力量及骨折事件的影响。方法采用前瞻性研究，94例废用性肌萎缩症患者按随机数字表分为对照组（运动疗法）与观察组（运动疗法＋肠内营养制剂＋维生素D），每组各47例。比较干预前及干预3个月、6个月及1年后两组患者肌肉力量、骨骼肌指数、6米步行速度的变化，同时记录两组发生跌倒及骨折事件。结果在干预6个月．72．1年后，观察组肌肉力量、骨骼肌指数、6米步行速度改善效果均优于对照组，差异有统计学意义（P〈0.05）；干预1年，观察组跌倒（4．3％）及骨折（0％）发生率均低于对照组（21．3％、12．8％），差异有统计学意义（P〈0．05）。结论肠内营养制剂联合维生素D可以有效改善老年骨骼肌废用性肌萎缩症患者的肌肉力量及骨骼肌指数，提高患者步行速度，降低跌倒及骨折发生率，具有一定的临床推广应用价值。Objective To investigate the effect of enteral nutrition combined with vitamin D on muscle strength and fracture events in elderly patients with skeletal muscle. Methods A prospective study of 94 patients with skeletal muscle reduction were included in the control group （ exercise therapy ） or observation group （ exercise therapy ＋ enteral nutrition ＋ vitamin D ） according to random number table, each group with 47 cases. The changes of muscle strength, skeletal muscle index and walking speed of 6m were compared before and 3 months, 6 months and 1 year after intervention. At the same time, the fall and fracture events were recorded. Results At 6 months and 1 year after intervention, the improvement effects of muscle strength, skeletal muscle index and 6m walking speed in the observation group were better than those in the control group, the difference was statistically significant （ P〈0.05 ） . At the same time, the incidence of fall （ 4.3% ） and fracture （ 0 ） in the observation group was lower than that in the control group （ 21.3%, 12.8% ） for one year. The difference was also statistically significant （ P〈0.05 ） . Conclusion Enteral nutrition combined with vitamin D can effectively improve the muscle strength and skeletal muscle index of elderly patients with skeletal muscle reduction, improve patient walking speed, reduce the incidence of falls and fractures, and has some clinical application value.
