摘 要:目的探讨刀锋技术(BLADE技术)在自由呼吸导航心脏磁共振(CMR)T2WI中的应用价值。方法对27例患者分别进行自由呼吸导航CMRBLADE-T2WI及常规TSE—T2WI扫描,记录扫描时间,测量图像信噪比(SNR)、心肌血池对比噪声比(CNR)及心脏边缘锐利度,采用配对t检验统计分析。由2位有经验的放射科医师对图像的整体质量、心脏结构显示进行4分度评分,评分结果采用Wilcoxom符号秩和检验统计分析。结果2种方法的扫描时间无统计学显著性差异[BLADE/TSE,(101.07±13.41)s/(95.04±13.29)S,P〉0.05];BLADE—T2WI序列的SNR与CNR(121.23±44.34/107.38±43.17)与TSE-T2WI的SNR与CNR(116.83±40.22/99.23±39.20)相比,差异无统计学意义(P〉0.05);BLADE-T2WI序列的心脏边缘锐利度值高于传统的TSE-T2WI,分别为0.43±0.11,0.31±0.90,差异有统计学意义(P〈0.05)。BLADE—T2WI序列的整体图像质量及心脏结构显示评分均高于TSE-T2WI序列(P〈0.05)。结论BLADE技术有助于提高自由呼吸导航CMR T2WI序列的图像质量,可应用于屏气效果差及自由呼吸导航常规TSE—T2WI图像欠佳的患者。Objective To investigate the value of BLADE technique applied in free-breathing respiratory navigated cardiac magnetic resonance (CMR) T2-weighted imaging.Methods 27 patients underwent free-breathing respiratory navigated BLADE-T2WI and conventional TSE-T2WI respectively.The data acquisition time was recorded for both the methods. The signal to noise ratio (SNR), contrast to noise ratio (CNR) between myocardium and blood pool, and the sharpness of heart edge were measured quantitatively and compared using the paired t-test statistically.The overall imaging quality and the visualization of heart structure were evaluated by two experienced radiologists using a 4-point scale and the results were analyzed statistically using the Wilcoxon signed-ranks test. Results No significant difference was found between the data acquisition time of the two methods [-BLADE/TSE, (101.07 ± 13.41) s/( 95.04 ± 13.29) s, P 〉0.05]. The SNR and CNR of BLADE-T2 WI were similar to those of the TSE-T2WI (121.23±44.34/116.83±40.22, 107.38±43.17/ 99.23 ± 39.20;P〉0.05 for both).BLADE-T2 WI was superior to conventional TSE-T2WI in term of the sharpness of the heart edge (0.43±0.11/0.31 ± 0.90, P 〈0.05 ). The overall imaging quality and the display of heart structure of BLADE-T2 WI were significantly higher than those of TSE-T2WI (P(0.05).Conclusion BLADE is useful for improving the image quality of the free-breathing respiratory navigated CMR T2 WI.h can be applied to patients who can't hold their breath well or have obvious artifacts in conventional free-breathing CMR T2WI.
