加味独活寄生合剂对膝关节骨性关节炎关节液中IL-1、NO、Sox9及CollagenⅡ的影响

摘　　要：目的:观察加味独活寄生合剂对膝关节骨性关节炎(膝骨关节炎)关节液中白细胞介素-1(IL-1)、NO、Sox9及胶原蛋白(CollagenⅡ)的影响,探讨加味独活寄生合剂对膝骨关节炎关节软骨的保护机制。方法:选择湖南中医药大学第一附属医院骨伤科门诊的膝骨关节炎患者60例,按照随机数字表法,随机对照分为两组,加味独活寄生合剂组(治疗组)和独活寄生合剂组(对照组),两组患者口服药物均为每次62.5mL,每天2次,治疗时间为6周,观察两组患者用药前后视觉模拟疼痛评分(VAS)和西安大略和麦玛斯特大学骨关节炎指数(WOMAC)的改变,同时抽取治疗前后关节液,采用ELISA法检测IL-1、NO、Sox9及CollagenⅡ并进行本组治疗前后和两组间对比。结果:治疗后两组患者VAS评分和WOMAC评分均明显降低(P<0.05),且加味独活寄生合剂组临床效果改善更佳(P<0.05);治疗后两组关节液中IL-1、NO含量明显降低(P<0.05),Sox9及CollagenⅡ含量明显升高(P<0.05),且两组组间比较差异有统计学意义(P<0.05)。结论:加味独活寄生合剂能够有效的缓解膝骨关节炎患者的临床症状,同时能够降低炎性因子IL-1、NO,增加Sox9、CollagenⅡ蛋白的含量,促进关节软骨的增殖修复,这也许是加味独活寄生合剂治疗膝骨关节炎的分子机制。Objective： To observe the effects of Jiawei Duhuo Jisheng Mixture on IL-1, NO, Sox9, and Collagen II in synovial fluid of knee osteoarthritis, and to discuss the protection mechanism of Jiawei Duhuo Jisheng Mixture on knee osteoarthritis＇s articular cartilage. Methods： According to the random number table method, 60 patients selected were randomly divided into 2 groups, Jiawei Duhuo Jisheng Mixture treatment group and Duhuo Jisheng Mixture control group, from the Department of Orthopedic Trauma in the First Hospital of Hunan University of Chinese Medicine. Both groups were taken 62.5mL orally each time, twice a day, 6 weeks as a course. The changes of VAS scores and WOMAC scores before and after treatment were observed in both groups, meanwhile, the joint fluid was extracted before and after treatment, IL-1, NO, Sox9, Collagen 1I were detected by ELISA and compared between groups. Results： Compared with before treatment, the VAS scores and WOMAC scores of two groups were significantly lower after treatment （P〈0.05）, and the clinic effect of Jiawei Duhuo Jisheng Mixture improved better （P〈0.05）. Compared with before treatment, the contents of IL-1 and NO in two groups were decreased and the contents of Sox9 and Collagen 1I were increased after treatment （P〈0.05）, the comparison between the two groups was statistically significant （P〈0.05）. Conclusion： Jiawei Duhuo Jisheng Mixture can effectively relieve the clinical symptoms of patients with knee osteoarthritis, at the same time, it can reduce the inflammatory factors IL-1, NO, and increase Sox9, Collagen Ⅱ protein contents. Promoting the cell proliferation and repair of articular cartilage may be the molecular mechanism of Jiawei Duhuo Jisheng Mixture in the treatment of knee osteoarthritis.
