穴位注射脉络宁对肛肠手术患者术后镇痛及相关并发症影响的临床研究

摘　　要：目的观察麻醉药复合脉络宁注射液骶管麻醉并配合穴位注射的有效性及对麻醉相关并发症发生率的影响。方法患者200例随机分为两组。对照组使用2%利多卡因加盐酸罗哌卡因10 m L加0.9%氯化钠注射液骶管内注射。治疗组在此基础上在长强穴、腰腧穴分别注射脉络宁注射液。记录两组患者术后第1,2,3,5日疼痛视觉模拟量表(VAS)评分。骶管麻醉相关副作用发生率(尿潴留、神经损伤相关并发症),及疼痛减轻程度满意度。结果治疗组第1次感受到手术部位疼痛的时间缩短,住院期间疼痛VAS评分降低,镇痛药使用累积量下降,疼痛减轻程度满意度增加以及住院花费下降(P<0.05)。结论长强穴、腰腧穴注射脉络宁能提高肛肠手术患者术后镇痛效果。Objective： To observe the efficacy of local anesthetic compound Venorine Injection combined with acupoint injection and its effect on the incidence of anesthesia related complications. Methods： 200 cases were randomly divided into 2 groups： the treatment group and the control group,with 100 cases in each. The control group received sacral injection of 2% lidocaine ＋ 0.75% ropivacaine hydrochloride ＋ 0.9% NS. On this base,the treatment group were injected in Changqiang acupoint and lumbar acupoint,respectively. The VAS scores on the 1st,2nd,3rd and 5th days were recorded,as well as the incidence of sacral anesthesia related side effects（urinary retention,nerve injury related complications） and satisfaction degree of pain relief were satisfactory. Results： The first time the treatment group felt the pain of operation sites was shortened;during hospitalization,the pain VAS scores decreased;the cumulative dosage of analgesics decreased;the satisfaction degree of pain relief increased,and the cost of hospitalization decreased. The improvement rate of each index in the treatment group was higher than that in the control group（P〈0.05）. Conclusion： Changqiang point and lumbar acupoint injection can improve the postoperative analgesic effect of patients undergoing anorectal surgery.
