摘 要:目的探讨IL-1、IL-6含量及其基因多态性与椎间盘源性腰痛患者椎间盘MRI信号值的关联研究。方法选择130例椎间盘源性腰痛患者。根据是否发生高信号区(HIZ)分成HIZ组94例,不伴HIZ组36例,收集患者的临床、生化及影像学资料,行腰椎后路椎间盘髓核切除采取标本进行IL-1、IL-6含量测定,并检测IL-1(rs1800587)和IL-6(rs1800797)基因多态性。结果IL-1及IL-6含量HIZ组较不伴HIZ组高,IL-1(rs1800587)基因型分布频率比较,TT基因型HIZ组显著高于不伴HIZ组,IL-1(rsl800587)基因型分层比较显示,TT基因型IL-1含量最高,组间比较差异有统计学意义(P〈0.05),通过直线相关分析,T2相对比值IL-1含量的相关系数:r=-0.78,P=0.03,T2相对比值IL-6含量的相关系数:r=-0.82,P=002。结论IL-1、IL-6在椎间盘源性腰痛病变过程中起着重要作用,与椎间盘T2信号值呈负相关关系,IL-1(rs1800587)基因多态性对椎间盘退变发病过程有一定影响。Objective To discuss the association of discogenic low back pain with intervertebral disc MRI signal and IL-1 and IL-6 gene polymorphisms. Methods Collect clinical, biochemical and imaging data of all 130 patients with discogenic lumbago were divided 94 patients into HIZ group and 36 patients into non HIZ group. All patients were performed lumbar posterior intervertebral disc nucleus pulposus resection, the nucleus pulposus fiber ring samples IL-1, IL-6 content determination were taken, and the detection of IL-1 (rs1800587) and IL-6 (rs1800797) gene polymorphism were done. Results IL-1 and IL-6 contents in HIZ group were higher than non HIZ group. IL-1 ( rs1800587 ) genotype distribution frequency comparison showed TT genotype in HIZ group were higher than non HIZ group. IL-1 (rs1800587) genotypic stratification comparison showed IL-1 contents in TT genotype was highest, so the comparison between groups was statistically significant (P〈0.05) . By the linear correlation analysis, T2 ratio and the correlation coefficient of content of IL-1: r=0.78, P=0.03, T2 ratio and correlation coefficient of content of IL-6: r=0.82, P=0.02 ) . Conclusion IL-1, IL-6 in intervertebral disc source sex lumbago disease plays an important role in the process, and negatively correlated with intervertebral disc T2 signal value. IL-1 ( rs1800587 ) gene polymorphisms have some impact on the process ofintervertebral disc degeneration.
