基于PACS探讨皮下脂肪含量与脂肪肝的相关性

摘　　要：目的探讨皮下脂肪含量与肝脏脂肪变性程度的相关性。方法在医学影像归档与通信系统（PACS）上回顾性测量行腹部CT扫描患者的腰围、皮下脂肪面积和厚度，以及肝脏和脾的CT值并求出两者的比值（以下简称：CT比值），并借助统计软件分析其相关性。结果148例患者平均皮下脂肪面积为（243．07±68．40）cm2，腰围为（124．82±23．57）cm，两者相关系数r=-0．856，皮下脂肪平均厚度（1．87±0．53）cm，肝脏CT值为（48．93±10．39）Hu，脾脏CT值为（46．71±2．84）Hu。皮下脂肪厚度与CT比值的相关系数r=－0．169，皮下脂肪面积与CT比值的相关系数r=-0．526。结论皮下脂肪含量与肝脂肪变性有中度相关性，但腹部皮下脂肪厚度与肝脏脂肪变性无相关性，可见通过皮下脂肪含量来推测肝脏脂肪变性比单纯脂肪厚度可能更有价值。Objective To investigate the correlation between subcutaneous fat content and the degree of liver steatosis. Methods The waist circumference, subcutaneous fat area and thickness, and the CT values of liver and spleen of the patients, who performed abdominal CT scan, were retrospectively measured in the PACS workstation, and the ratio of the CT values were calculated （ hereinafter referred to as： CT ratio ） . The correlation were analyzed by statistical software. Results Of the 148 patients, the average area of subcutaneous tht was （ 243.07 ± 68.40 ） cm2, the average waist circumference was （ 124.82 ± 23.57 ） cm, the correlation coefficient r=0.856, the average thickness of subcutaneous fat was 1.87 ±0.53cm, the CT value of liver was （ 48.93 ± 10.39 ） Hu, the CT value of spleen was （ 46.71 ± 2.84 ） Hu. The correlation coefficient between subcutaneous fat thickness and CT ratio was -0.169, and the correlation coefficient between subcutaneous fat area and CT ratio was -0.526. Conclusion There is a significant correlation between subcutaneous fat content and waist circumference. But there is no obvious correlation between abdominal subcutaneous fat thickness and liver steatosis. This shows that it may be more valuable through subcutaneous fat content to speculate the degree of liver steatosis than subcutaneous fat thickness alone.
