摘 要:目的分析新生儿败血症患儿血清25-羟基维生素D与抗菌肽-37(LL-37)的相关性及新生儿25-羟基维生素D的影响因素,为临床治疗新生儿败血症及补充维生素D提供依据。方法采集149例新生儿静脉血,其中76例败血症新生儿为败血症组,73例无败血症新生儿为对照组,检测两组新生儿25-羟基维生素D及LL-37水平,分析两者的相关性及新生儿25-羟基维生素D的影响因素。结果败血症组患儿血清25-羟基维生素D、抗菌肽LL-37水平均低于对照组(P=0.00);败血症组新生儿血清25-羟基维生素D和LL-37呈正相关(r=0.323,P=0.000);对照组新生儿25-羟基维生素D和LL-37无相关性(r=0.206,P=0.080)。单因素分析显示,不同民族、胎龄、出生体重、母亲年龄、母亲学历、孕期补充维生素D情况以及出生季节的25-羟基维生素D水平差异均有统计学意义(P均<0.05);多因素回归分析表明新生儿25-羟基维生素D水平与出生季节、母亲孕期补充维生素D情况以及胎龄相关(P<0.05)。结论新生儿败血症患儿血清25-羟基维生素D和LL-37水平均降低,且两者呈正相关。影响新生儿25-羟基维生素D水平的因素为出生季节、母亲孕期补充维生素D情况以及出生胎龄。Objective To analyze the level and correlation of serum 25-hydroxyvitamin D and antimicrobial peptide in neonatal septicemia and the influence factors of 25-hydroxyvitamin D in neonates,and to provide the basis for clinical treatment of neonatal sepsis and vitamin D supplementation. Methods The venous blood samples of 149 were collected from neonates,including 76 neonatal of septicemia(case group)and the 73 non-neonatal of septicemia neonates(control group). The levels of 25-hydroxy vitamin D and antimicrobial peptide-37 were measured. The correlation between the two groups and the factors affecting the 25-hydroxyl vitamin D of the newborn were analyzed. Results Both of the levels of 25-hydroxy vitamin D(8.18±4.28)ng·mL^(-1) and antibacterial peptide-37(252.05±184.37)pg·mL^(-1) in neonatal sepsis were significantly lower than those in the control group. There was a positive correlation between vitamin D and antibacterial peptide-37 in the case group. One-way ANOVA analyzing showed that there was statistically difference between ethnicity,gestational age,birth weight,the age of the mother,education history of the mother,vitamin D supplementation during pregnancy,the birth season and 25-hydroxy vitamin D levels. The levels of 25-hydroxy vitamin D correlate to the birth season,vitamin D supplementation during pregnancy,gestational age by multi factor regression analysis. Conclusion The levels of serum25-hydroxyl vitamin D and antimicrobial peptide decreased in neonates with septicemia,and there was a positive correlation between them. The factors affecting the level of 25-hydroxyvitamin D in neonates were the birth season,vitamin D supplementation during pregnancy and gestational age.
