双钢板固定植骨术结合经皮复位有限固定术在Schatzker Ⅴ/Ⅵ型胫骨平台骨折中的应用

摘　　要：目的探究双钢板固定植骨术结合经皮复位有限固定术在SchatzkerⅤ/Ⅵ型胫骨平台骨折中的应用。方法选取本科2014年5月至2015年10月收治的25例SchatzkerⅤ/Ⅵ型胫骨平台骨折患者,视患者软组织的损伤情况分别进行经皮复位有限固定术和双钢板固定植骨术治疗,术后随访6~12个月,观察膝关节功能评分、愈合情况和并发症发生情况。结果 25例患者均完成随访,Rasmussen评分优良率达92%,5例出现张力水疱,1例出现浅表感染,1例出现排斥反应,1例出现创伤性关节炎导致恢复较差,采取对症治疗后均治愈出院。患者均愈合良好,无骨折畸形愈合、延迟愈合、骨筋膜室综合征、创口溃烂感染、下肢静脉血栓形成等并发症。结论双钢板固定植骨术结合经皮复位有限固定术在SchatzkerⅤ/Ⅵ型胫骨平台骨折中能够减少膝关节周围软组织损伤,对膝关节起到生物学固定的作用,促进膝关节功能恢复,减少并发症发生,值得在临床推广应用。Objective To study the application of double plate fixation combined with percutane ous reduction and limited fixation in Schatzker V/Ⅵ tibial plateau fractures. Methods A total of 25 patients with Schatzker V/Ⅵ tibial plateau fractures in our hospital from May 2014 to October 2015 were selected. The patients were treated with double plate fixation and bone grafting combined with percutaneous reduction and fixed fixation. All patients were followed up for 6 to 12 months to observe the function score, healing and complication of the knee joint. Results All of the patients were followed up completely, the rasmussen score excellent rate achieved to 92%, 5 cases occurred tension blisters, 1 case occurred superficial infection, 1 case occurred reiection, 1 case occurred traumatic arthritis caused by poor recovery. All cases were cured and discharged after symptomatic treatment. Patients were well healed, no fracture malunion, delayed healing, osteofascial compartment syndrome, wound ulceration infection, lower extremity venous thrombosis and other complications occurred. Conclusion Double plate fixation combined with percutaneous reduction and limited fixation in the Schatzker V /Ⅵ tibial plateau fracture can reduce the soft tissue iniury around the knee, play a physiological role in the knee joint, promote the recovery of knee function and reduce the complications, and is worthy of clinical application.
