摘 要:目的分析6个月龄内肺炎患儿的肺炎支原体(MP)、肺炎衣原体(CP)、沙眼衣原体(CT)、解脲支原体(UU)感染分布和临床特点。方法收集2014年1月至2015年4月住院治疗的6个月龄内社区获得性肺炎婴儿689例,入院行胸片/CT检查,采用凝集法进行肺炎支原体(MP)、肺炎衣原体(CP)IgM抗体检测;检测外周血实验室指标;吸取痰标本做细菌培养,用PCR法对沙眼衣原体(CT)、解脲支原体(UU)进行核酸检测。结果 6月龄内肺炎婴儿MP/CP/CT/UU阳性检出213例(30.9%),阳性确诊116例(16.8%),MP、CP、CT、UU的阳性检出率分别为20.2%、10.3%、4.9%、6.5%,确诊率分别为7.8%、3.2%、4.9%、6.5%,阳性检出率整体差异均具有统计学意义(χ2=13.818,P=0.003)。1~3个月龄肺炎患儿MP/CP/CT/UU阳性检出和确诊率较高(61.5%,60.4%),女婴阳性检出和确诊率明显大于男婴(χ2=5.840,5.760;P=0.012,0.016)。61.2%的阳性确诊婴儿的胸片/CT表现为斑片状阴影,67.2%出现肺部湿性啰音,69.8%出现血小板异常升高。结论 MP、CP、CT、UU是6月龄内婴儿获得性肺炎的常见病原体,早期诊断和合理使用抗生素治疗可以有效预防和控制支原体、衣原体的感染。Objective To analyze the distribution of Mycoplasma pneumoniae (MP), Chlamydia pneumonia (CP), Chlamydia trachomatis (CT), Ureaplasma urealyticum (UU) infection and clinical characteristics of children aged 0 to 6 months with pneumonia. Methods A total of 689 cases of pneumonia aged 0 to 6 months from April 2015 to January 2014 were collected to make examination of chest X-ray or CT, IgM antibody of MP and CP were detected by agglutination method, the laboratory index of peripheral blood, cultivate bacteria of sputum samples were test ed, CT, UU nucleic acid were detected with the PCR method. Result In infants aged 0 to 6 months with pneumonia, 213 cases (30. 9%) were positive for MP/CP//CT/UU and 116 cases (16.8%) were confirmed. The positive detection rates of IMP, CP, CT, UU were 20.2%, 10. 3%, 4.9%, 6.5% respectively, and the diagnosis rate was 7.8%, 3.2%, 4.9%, 6.5% re spectively, the difference of the positive detection rate was statistically significant(χ^2=2 13. 818, P 0. 003). MP/CP/CT/UU positive detection and the diagnosis rate (61.5G, 60. 4%) was high er in 1 to 3 months old infants with pneumonia. The rate of posi male infants was significantly higher than that of male infants rive detection and diagnosis of fe (χ^2=5.840, 5.760%P 0.012, 0. 016), 61.2% of the diagnosed infected infants showed patchy shadows with chest Xray or CT, wet rales were heard in the lung of 67.2- of the diagnosed infected infants, platelet in 69.8% of the diagnosed infected infants abnormal increased. Conclusion MP, CP, CT and UU are common pathogens in infants aged 0 to 6 months with acquired pneumonia. Early diagnosis and rational use of antibiotics can effectively prevent and control the infection of Mycoplasma and Chlamydia.
