摘 要:目的:探讨三维能量多普勒血流定量技术结合超声弹性成像在早期乳腺癌诊断及临床治疗指导中的应用价值。方法:选择医院收治的经组织病理检验证实的56例早期乳腺癌患者纳入早期乳腺癌组,另选取同期明确诊断为乳腺良性病变的40例患者纳入良性病变组,以及50名体检健康女性纳入健康对照组,3组均接受三维能量多普勒超声以及超声弹性成像检查,对患者乳腺及肿块内血管进行血流定量分析处理,比较乳腺癌与良性病变患者血管相关参数及两种超声检查技术联合应用在早期乳腺癌诊断中的价值。结果:早期乳腺癌患者平均血管形成指数(VI)、血流指数(FI)及血管形成-血流指数(VFI)明显高于良性病变患者,良性病变患者VI、FI及VFI明显高于健康对照组,且3组的弹性成像弹性评分比较,其差异有统计学意义(F=1.142,F=2.843,F=2.599;P<0.05);乳腺癌患者肿块体积越大则肿瘤组织彩色像素密度(CPD)越高,二者具有正性相关关系(r=0.553,P<0.05)。结论:采用三维能量多普勒血流定量技术结合超声弹性成像可实现早期乳腺癌的筛查与诊断,并且二者为临床诊断及治疗提供的信息不同,联合检查通过对相关指标的采集、分析对临床治疗提供指导价值。Objective: To discuss the application value of blood flow quantitative technique of three-dimensional(3D) energy Doppler combined with ultrasound elastography in the diagnosis and clinical treatment of early breast cancer. Methods: 56 patients with early breast cancer, who were confirmed by histopathology, were divided into early breast cancer group, and 40 patients with benign breast lesions were divided into benign lesion group, and 50 healthy women were divided into health group. All of patients of three groups received the examinations of blood flow quantitative technique of 3D energy Doppler and ultrasound elastography. And the relevant parameter of blood vessel of patients with breast cancer and benign lesion, and the value of combined application of two kinds of ultrasound examination techniques in the early diagnosis were further compared. Results: The means of angiogenesis index(VI), blood flow index(FI) and angiogenesis-flow index(VFI) of patients with early breast cancer were significantly higher than those of patients with benign lesions. And the VI, FI and VFI of benign lesion group were significantly higher than those of health group. And the differences of elasticity score of elastography among three groups were significantly(F=1.142, F=2.843, F=2.599, P〈0.05). The increasing of color pixel density of tumor tissues was correlation with the tumor volume of patients with breast cancer, and it is positive correlation between them(r=0.553, P〈0.05). Conclusion: To adopt blood flow quantitative technique of 3D energy Doppler combined with ultrasound elastography can achieve screening and diagnosis of early breast cancer, and they can provide different information for the clinical diagnosis and treatment. The combined examination can provide guidance value for clinical treatment through collecting and analyzing relevant indicators.
