氟比洛芬酯缓解膀胱镜检查疼痛的临床研究

摘　　要：目的探讨氟比洛芬酯超前镇痛在缓解膀胱镜检查疼痛的效果。方法选择80例门诊男性膀胱镜检查患者,按均衡随机化分组的方法分为氟比洛芬酯组(F组)和对照组(C组),每组40例。F组于检查前10 min静脉注射氟比洛芬酯50 mg,C组于检测前10 min静脉注射生理盐水5 ml。术中两组患者均用丁卡因凝胶注入尿道局麻下进行操作。观察并比较两组患者术中(T_0)、术后1 h(T_1)、2 h(T_2)、4 h(T_3)、8 h(T_4)内视觉模拟评分(VAS),术前、术中、术后平均动脉压(MAP)、心率(HR)的改变,一次插镜成功率及插镜时间。结果 F组患者术中及术后各时段疼痛强度评分均低于C组(P<0.05),且C组术中、术后MAP、HR升高更明显(P<0.05)。另外,F组一次插镜成功率明显高于C组,且插镜时间明显缩短(P<0.05)。结论氟比洛芬酯超前镇痛可有效缓解膀胱镜检查术中及术后疼痛,维持血流动力学稳定,且能提高插镜成功率,缩短插镜时间。Objective To investigate the effect of preemptive analgesia of flurbiprofen axetil on pain in cystoscopy. Methods Totally 80 cases of male outpatients undergoing cystoscopy were divided into flurbiprofen axetil group （group F） and control group （group C） by randomization method,40 cases in each group. Patients in group F were injected with 50 mg of flurbiprofen axetil at 10 rain before cystoscopy, while group C were injected 5 ml of normal saline. The tetracaine gel was put into urethras for local anesthesia during operation. Changes in VAS, mean arterial pressure （MAP） and heart rate （HR） at TII （ during operation） , TI （ 1 h after operation） , T2 （ 2 h after operation） , T3 （4 h after operation） and T4 （ 8 h after operation） were recorded,and the one-time success rate and time of insertion of cystoscopy were observed and compared between the 2 groups. Results The VAS in group F was lower than that in group C （ P 〈 0. 05 ）. The MAP and HR in group C were increased more significantly （ P 〈 0. 05 ）. The one-time success rate in group F was higher than that of group C （ P 〈 0. 05 ） , and the time of insertion of cystoscopy in group F was shorter （ P 〈 0. 05 ）. Conclusion Preemptive analgesia with flurbiprofen axetil can effectively alleviate the pain during and after cystoscopy,maintain the stability of hemodynamics,improve the success rate and shorten the time of insertion of cystoscopy.
